IT IS HEREBY CERTIFIED that this Certificate of Public Convenience and Necessity, Certificate No. 353, is issued to Level 3 Communications, LLC. This Certificate authorizes Level 3 to provide Title 61 regulated telecommunications services, including basic exchange services, within certain parts of the state of Idaho. The Commission approved Level 3s Certificate by Order No. 27855 issued December 31, 1998, and authorized Level 3 to provide Title 61 services in the respective service areas of U S WEST Communications and GTE Northwest Incorporated, as depicted in the map attached to this Certificate as Exhibit A. This Certificate is predicated upon and is issued pursuant to the findings of the Commission in Order No. 27855.
