On May 8,2012,Liberty-Bell Telecom,LLC dba DISH Network Phone &Internet
notified the Commission that it had changed its name to dishNET Wireline L.L.C.The
Company ceased doing business as DISH Network Phone &I±iternet effective May 21,2012.
The name change is not a result of corporate reorganization and does not affect Company
operations and customers.A copy of the Secretary of State’s Amended Certificate of Authority
reflecting the name change was included with the Company’s request for name change.
In December 2010,the Commission issued Order No.32153 granting Liberty-Bell
Telecom,LLC a Certificate of Public Convenience and Necessity to provide facilities-based
local exchange,switched exchange access and non-facilities-based interexchange
telecommunications services in Idaho.In January 2011,Certificate No.500 was issued to
Liberty-Bell.
In March 2011,the Commission issued Order No.32213 approving a name change
from Liberty-Bell Telecom,LLC to Liberty-Bell,LLC dba DISH Network Phone &Internet,and
issued First Amended Certificate No.500.
Based on our review of the Application,the Commission finds that it is reasonable to
amend Certificate of Public Convenience and Necessity No.500 to reflect the change in name
from Liberty-Bell,LLC dba DISH Network Phone &Internet to dishNET Wireline L.L.C.
ORDER
IT IS HEREBY ORDERED that Certificate of Public Convenience and Necessity
No.500 is amended to reflect the name change from Liberty-Bell,LLC dba DISH Network
Phone &Internet to dishNET Wireline L.L.C.
THIS IS A FINAL ORDER.Any person interested in this Order may petition for
reconsideration within twenty-one (21)days of the service date of this Order with regard to any

matter decided in this Order in Case No.LBT-T-10-01.Within seven (7)days after any person
has petitioned for reconsideration,any other person may cross-petition for reconsideration.See
Idaho Code §61-626 and 62-619.
