Necessity, Certificate No. 386, is issued to Integra Telecom of Idaho, Inc. This Certificate
authorizes Integra to provide local exchange and interexchange services within the state of Idaho.
The Commission approved Integra’s Certificate by Order No. 28585 issued December 12, 2000,
and authorized Integra to provide telecommunications services in the state of Idaho. This
Certificate is predicated upon and is issued pursuant to the findings of the Commission in Order
No. 28585.
