changed the name under which it does business from Mitel NetSolutions,Inc.to Mitel Cloud
Services,Inc.Mitel provided an Idaho Certificate of Authority from the Idaho Secretary of
State’s office.
IT IS HEREBY CERTIFIED that this Amended Certificate of Public Convenience
and Necessity,Certificate No.499,is issued to Mitel Cloud Services,Inc.This Certificate
authorizes Mite!to provide local exchange telecommunications services within the State of
Idaho.The Commission approved this name change to Certificate No.499 by Order No.33263
in Case No.ITN-T-09-01.
