On April 19,2018,Intermountain Infrastructure Group,LLC ("IIG"or "Company")
filed an Application with the Commission,per Procedural Order No.32277 and IDAPA
31.01.01.114,for registration as a facilities-based provider of non-basic local
telecommunications services within the State of Idaho.Application at 1.
NOTICE OF APPLICATION
YOU ARE HEREBY NOTIFIED that IIG is a corporation organized in the State of
Delaware and lists its principal place of business as Burlingame,California.Application at 1.
Idaho's Secretary of State certified that IIG was authorized to transact business in Idaho as of
March 26,2018.Id;Exhibit A.The Company lists Northwest Registered Agent LLC,784 S
Clearwater Loop,Suite B,Post Falls,Idaho 83854,as its registered agent in Idaho.Id at 2.
YOU ARE FURTHER NOTIFIED that IIG is seeking authority to be a facilities-
based provider of non-basic local telecommunications services in the State of Idaho.
YOU ARE FURTHER NOTIFIED that IIG "agrees to comply with rules applicable
to competitive providers of non-basic local telecommunications services"in Idaho.Id.Also,
Intermountain agrees "it will be subject to numbering conservation measures,including
mandatory one thousand block pooling,donation back to the pool of unused or underutilized
thousand number blocks,and submission of applicable utilization/forecast reports at the one
thousand block level for each rate center within its service territory."Id at 3.

YOU ARE FURTHER NOTIFIED that the Commission has determined that the
public interest may not require a formal hearing in this matter and will proceed under Modified

Procedure pursuant to Rules 201 through 204 of the Idaho Public Utilities Commission's Rules
of Procedure,IDAPA 31.01.01.201 through .204.The Commission notes that Modified
Procedure and written comments have proven to be an effective means for obtaining public input
and participation.
YOU ARE FURTHER NOTIFIED that any person desiring to state a position on this
Application may file a written comment in support or opposition with the Commission on or
before July 30,2018.The comment must contain a statement of reasons supporting the
comment.Persons desiring a hearing must specifically request a hearing in their written
comments.Written comments concerning this Application shall be mailed to the Commission

YOU ARE FURTHER NOTIFIED that all proceedings in this case will be held
pursuant to the Commission's jurisdiction under Title 62 of the Idaho Code and specifically
Idaho Code §§62-604 and 615.The Commission may enter any final Order consistent with its
authority under Title 62 and Commission Rule of Procedure 114,IDAPA 31.01.01.114.
YOU ARE FURTHER NOTIFIED that all proceedings in this matter will be
conducted pursuant to the Commission's Rules of Procedure,IDAPA 31.01.01.000 et seq.

IT IS HEREBY ORDERED that this Application be processed under Modified
Procedure,IDAPA 31.01.01.201-.204.Persons interested in submitting written comments or
protesting the use of Modified Procedure must do so no later than July 30,2018.
