Authorization Code - A pre-defined series of numbers to be dialed by the Customer or Authorized User upon access to the Carrier' Travel Service network to identify the caller and validate thecaller's authorization to use the services provided. Authorized User - A person, firm , corporation, or any other entity authorized by the Customer to utilize the Carrier's service. Commission - The Idaho Public utilities Commission unless otherwise clearly indicated by the context. Company or Carrier - IdealDial Corporation unless otherwise clearly indicated by the context. Customer - The person, firm, corporation or other entity which orders, cancels , amends or uses service and is responsible for paYment of charges and compliance with the Company's price list. Equal Access - Where the local exchange company central office provides interconnection to interexchange carriers with FeatureGroup D circuits. In such end offices, customers presubscribe their telephone line ( s) to their preferred interLATA carrier. IdealDial - Used throughout this price list to refer to IdealDialCorporation LEC - Local Exchange Company. Personal Identification Number (PIN) See Authorization Code. PINs may also be used in conjunction with shared 800 service to uniquely identify the terminating location for an incoming call. Upon dialing a shared 800 number, the caller enters the PIN number associated with the party they are trying to reach. Serving wire Center - A specified geographic point from which thevertical and horizontal coordinate is used in calculation of airline mileage. Switched Access Origination/Termination - Where access between the. customer and the interexchange carrier is provided on local exchange company Feature Group circuits and the connection to the customer is a LEC-provided business or residential access line. The cost of switched Feature Group access is billed to the interexchange carrier. Travel Card Call - A service whereby the Customer or Authorized User dials all of the digits necessary to route and bill a call placed from a location other than his/her residence or normal placeof business. Service is accessed via a "1-800" or other access code dialing sequence. V & H Coordinates - Geographic points which define the originating and terminating points of a call in mathematical terms so that the airline mileage of the call may be determined. Call mileage may be used for the purposed of rating calls. IdealDial installs, operates , and maintains the communications services provided hereinunder in accordance with the terms andcondi tions set forth under this price list. IdealDial may actas the Customer'agent for ordering access connectionfacili ties provided by other carriers or entities , whenauthorized by the Customer, to allow connection of aCustomer's location to the IdealDial network. The Customershall be responsible for all charges due for such servicearrangement. The Company's services and resold facilities are provided on a monthly basis unless otherwise provided, and are available twenty-four hours per day, seven days per week. Limi tat ions Service is offered subject to the availability of the necessary facilities and equipment and subject to the provisions of this price list. IdealDial reserves the right to discontinue or limi t service when necessitated by conditions beyond its control, or when the Customer is using service in violation of provisions of this price list, or in violation of the law. The Company does not undertake to transmit messages, but offers the use of its facilities when available, and will not be liable for errors intransmission or for failure to establishconnections. Limi tat ions, (Cont ' d. ) Use All services and resold facilities provided underthis price list are directly or indirectlycontrolled by IdealDial and the Customer may not transfer or assign the use of service or facilities wi thout the express wri tten consent of theCompany. Such transfer or assignment shall only apply where there is no interruption of the use or location of the service or facilities. Prior wri tten permission from the Company is required before any assignment or transfer. All regulations and conditions contained in this price list shall apply to all such permitted assignees ortransferees, as well as all conditions of service. presubscribed service is available from equal access locations only. Travel service is available from any originating location in the state. Services provided under this price list may be used for any lawful purpose for which the service is technically suited. Payment for Service All charges due by the Customer are payable to IdealDial or any agency duly authorized to receive such paYments. Any objections to billed charges must be reported to the Company wi thin thirty (30) days of the invoice date. Adjustments toCustomer'bills shall be made to the extent that circumstances exist which reasonably indicate that such changes are appropriate. Taxes Late Payment Fees A late paYment fee of 1. 5% per month will be charged on any past due balance. Any applicable late paYment fees will be assessed according to the terms and conditions of the Company or its billing agent and pursuant to Pennsylvania law. Ret urn Check Charge A return check charge of $20.00 will be assessed for checks returned for insufficient funds. Any applicable return check charges will be assessed according to the terms and condi tions of the Company or its billing agent and pursuant toPennsylvania law. Unless otherwise specified in this price list, all state andlocal taxes (i. e., gross receipts tax, sales tax, municipalutili ties tax) are listed as separate line items and are not included in the quoted rates. Cancellation Customers must provide thirty days written notification to IdealDial prior to cancellation. Customers are responsible for all charges, including fixed fees, which accrue up to thecancellation date. It shall be the obligation of the Customer to notify Company immediately of any interruption in service for which a credit allowance isdesired by Customer. Before giving such notice, the Customer shall ascertain that the trouble is not wi thin his or her control, or is not in wiring or equipment, if any, furnishedby Customer. Interruptions caused by Customer-providedSubscriber-provided or Company-provided automatic dialing equipment are not deemed an interruption of service as defined herein since the Customer has the option of using the long distance network via LEC access. For purposes of credit computation every month shall be considered to have 30 days. For services with a monthly recurring charge, no credit shall be allowed for an interruption of continuous duration of lessthan twenty-four hours. For message rated toll services,credits will be limited to , at maximum , the price of the initial period of individual call that was interrupted. The Customer shall be credited for an interruption of one day (24 hours) or more at the rate of 1/30th of the monthly charge for the services affected for each day that the interruptioncontinues. Credit Formula: Credi t =A/30 A = outage time in days B = total monthly charge for affected service. The Company will make every effort to reserve "800" vani ty numbers on behalf of customers, but makes no guarantee or warrantee that the requested " 800" number ( s) will be available or assigned to the customer requesting the number. If a Customer accumulates undisputed past-due charges, the Company reserves the right not to honor the Customer's request for a change in 800 service to another carrier (e. g., "porting" of the 800 number), including a request for a Responsible Organization (Resp Org) change , until such time as all charges are paid in full. 800 numbers shared by more than one Customer, whereby indi vidual customers are identified by a unique Personal Identification Number, may not be assigned or transferred for use wi th service provided by another carrier. Subject to the limitations provided in Section 2.2, the company will only honor Customer requests for change in Resp Org or 800 service provider for 800 numbers dedicated to the sole use of that single Customer. Other Rules The Company reserves the right to discontinue service, limit service, or to impose requirements on Customers as required to meet changing regulatory rules and standards of the Idaho Public utilities Commission. SECTION 3 - SERVICE DESCRIPTIONS AND RATES IdealDial provides direct dialed (1+) service, inbound 800 number service , travel card service and prepaid card services for communications originating and terminating wi thin thestate of Idaho under terms of this price list. Operatorservices and directory assistance are not furnished by IdealDial. Operator services and directory assistance servicewill be provided and billed by the Company'underlyingcarrier. Customers are billed based on their use of IdealDialCorporation'network and services. Charges may vary by service offering, mileage band, class of call, time of day, day of week, and/or call duration. Timing of Calls Billing for calls placed over the IdealDial network is basedin part on the duration of the call as follows, unless otherwise specified in this price list: Call timing begins when the called party answers the call (i. e. when two way communications areestablished. Answer detection is based standard industry answer detection methods, including hardware and software answer detection. Chargeable time for calls ends when one of the parties disconnects from the call. Minimum call duration periods for billing purposes vary by service option and are specified on a perplan basis. For billing purposes usage after the initial period varies by service and is specified on a perplan basis. The Company will not bill for unanswered calls. When a Customer indicates that he/she was billed for an incomplete call, IdealDial will reasonably issue credit for the call. Calculation of Distance Usage charges for all mileage sensi ti ve products are based on the airline distance between serving wire centers associated with the originating and terminating points of the call. The serving wire centers of a call are determined by the area codes and exchanges of the origination and destination points. The distance between the Wire Center of the Customer' equipment and that of the destination point is calculated byusing the "V" and "H" coordinates found in BellCore' s V&H Tape and NECA FCC Tariff No. Step 1 - Step 2 - Step 3 - Step 4 - Step 5 - Step 6 - Formula: Obtain the "V" and "H" coordinates for the Wire Centers serving the Customer and the destinationpoint. Obtain the difference between the "V" coordinatesof each of the Wire Centers. Obtain the Difference between the "H" coordinates. Square the differences obtained in Step Add the squares of the "V" difference and difference obtained in Step "H" Di vide the sum of the square obtained in Step 4 byten (10). Round to the next higher whole number if any fraction results from the division. Obtain the square root of the whole number obtained in Step 5. Round to the next higher whole numberif any fraction is obtained. Option 3 service is limited to Customers whose anticipated usage charges exceed an average of $250.00 per month. Intrastate service is combined with intrastate service for purposes of determining usage volume and applicable discountrates. For new customers, IdealDial reserves the right to request copies of up to three ( 3) months in pastbills in order to verify Customer statements regarding anticipated usage volumes. A re-eval uation of an existing Customer's volume level and continued eligibility for option 3 rates may be performed at the Company'discretion. IdealDial reserves the right to periodically audit a Customer's average usage volumes based on pastbilling. Customers not meeting the required average volume level will be given the option of continuing service under some other qualifying plan or discontinuing the Company's service. Travel Service, (Cont'd. Travel Service - Option 2 (Debit Card) Travel Service - Option 2 is offered as a flat- rated travel card account service. Establishment of an account balance enti tIes the consumer to access the Carrier's network for a preset amount ofusage. Option 2 usage will be debited from the available account balance in full-minute incrementson real-time basis. Customers will interrupted with an announcement when the account balance is about to be exhausted. Debit cards containing account numbers and dialinginstructions are available in variousdenominations. The debit card is non-refundable and will expire on the date specified on the card or package in which the card is enclosed. Anyunused balances may be applied toward any replenishment amount. Unlike a deposit or advance paYment , the debit card account balance is not held against future paYment as all service is available for immediate consumption.
