CERTIFICATE NO. 364
IT IS HEREBY CERTIFIED that this Certificate of Public Convenience and Necessity is issued to Choctaw Communications, Inc. d.b.a. Smoke Signal Communications, Certificate No. 364, to authorize Choctaw to provide local telecommunications services and exchange services in U S WEST (north and south) and GTE service areas in. The Commission approved Choctaw’s Certificate by Order No. 28159 issued October 1, 1999. Order No. 28159 limits the areas in which Choctaw is authorized to provide services, as depicted in the map attached to this Certificate as Exhibit A. This Certificate is predicated upon and is issued pursuant to the findings of the Commission in Order No. 28159.
