IT IS HEREBY CERTIFIED that this Certificate of Public Convenience and Necessity is issued to Concert Communications Sales, LLC, Certificate No. 361, to authorize Concert Communications to provide local telecommunications services and exchange services in U S WEST Communications, Inc.’s and GTE Northwest Incorporated’s certificated service areas. The Commission approved Concert Communications’ Certificate by Order No. 28111 issued August 6, 1999. Order No. 28111 limits the areas in which Concert Communications is authorized to provide services, as depicted in the map attached to this Certificate as Exhibit A. This Certificate is predicated upon and is issued pursuant to the findings of the Commission in Order No. 28111.
