CERTIFICATE NO. 354

On August 9, 2000, Advanced TelCom Group, Inc. submitted a request to the Commission to change its name to Advanced TelCom, Inc. consistent with the Minute Entry dated August 25, 2000.
IT IS HEREBY CERTIFIED that this amended Certificate of Public Convenience and Necessity, Certificate No. 354, is issued to Advanced TelCom, Inc. to authorize Advanced TelCom to provide Title 61 regulated telecommunications services, including basic exchange services, within certain parts of the state of Idaho. In Order No. 27879 issued January 15, 1999, the Commission authorized Advanced TelCom to provide Title 61 services in the respective service areas of U S WEST Communications (now known as Qwest Corporation) and GTE Northwest Incorporated (now known as Verizon Northwest, Inc.), as depicted in the map attached to Certificate No. 354 as Exhibit A.
