IT IS HEREBY CERTIFIED that this Certificate of Public Convenience and Necessity is issued to CCCID, Inc., Certificate No. 358, to authorize CCCID to provide local telecommunications services and exchange services in U S WEST Communications, Inc.’s Idaho exchanges. The Commission approved CCCID’s Certificate by Order No. 28074 issued June 18, 1999. Order No. 28074 limits the areas in which CCCID is authorized to provide services, as depicted in the map attached to this Certificate as Exhibit A. This Certificate is predicated upon and is issued pursuant to the findings of the Commission in Order No. 28074.
