IT IS HEREBY CERTIFIED that this Certificate of Public Convenience and Necessity, Certificate No. 356, is issued to ICG Telecom Group, Inc. (ICG). This Certificate authorizes ICG to provide Title 61 regulated telecommunications services, including basic exchange services, within certain parts of the state of Idaho. The Commission approved ICGs Certificate by Order No. 27955 issued March 3, 1999, and authorized ICG to provide Title 61 services in the respective service areas of U S WEST Communications and GTE Northwest, as depicted in the map attached to this Certificate as Exhibit A. This Certificate is predicated upon and is issued pursuant to the findings of the Commission in Order No. 27955.
