IT IS HEREBY CERTIFIED that this Certificate of Public Convenience and Necessity, Certificate No. 340, is issued to Crystal Communications, Inc. This Certificate authorizes Crystal to provide Title 61 regulated telecommunications services, including basic exchange services, within certain parts of the state of Idaho. The Commission approved Crystals Certificate by Order No. 27503 issued May 11, 1998, and authorized Crystal to provide Title 61 services in the respective service areas of U S WEST Communications and GTE Northwest. This Certificate is predicated upon and is issued pursuant to the findings of the Commission in Order 27503.
