Form 481 with the Universal Service Administrative Corporation ("USAC")for designation as anEligibleTelecommunicationsCarrier,in accordance with 47 U.S.C.54.313 and 54.422.I haveincludedthesignedAffidavitpursuanttoCommissionOrder29841.Company,Study Area Code
472213,is a rural incumbent local exchange carrier (ILEC)as designated by the FCC,and iseligibletoreceivefederaluniversalservicehigh-cost support pursuant to the Code of FederalRegulations("CFR"),Title 47,Sections 54.313,54.422,and Part 36,sub-part F.

AFFIDAVIT OF BUSINESS OR CORPORATE OFFICER
The Idaho Public Utilities Commission Order No.29841 requires that Eligible Telecommunications Carriers(ETC)certify that it is compliant with applicable service quality standards and consumer protection rules;andETCsmustdemonstratetheabilitytoremainfunctionalinemergencies.In addition,the Commission must file
an annual certification with the USAC and the FCC that all federal high-cost support provided to ETCs withintheStateofIdahowillbeusedonlyfortheprovision,maintenance,and upgrading of facilities and services forwhichthesupportisintended.Accordingly,the undersigned states and verifies under oath the following:
1.I am an officer of Albion Telephone Company,Inc.an eligible telecommunications carrier for receivingfederaluniversalservicesupportundersection214(e)of the Telecommunications Act of 1996 in the
state of Idaho.
2.I am familiar with the Company's day-to-day operations in the state of Idaho and with the State'sservicequalitystandardsandconsumerprotectionrulesassetforthinCommissionOrderNo.29841.
3.Albion Telephone Company,Inc._is complying with applicable service quality standards and consumerprotectionrulesoftheFederalCommunicationsCommissionandtheIdahoPublicUtilitiesCommission.
4.I certify to the Commission that the Company is able to remain functional in emergencies as set forth inCommissionOrderNo.29841 and in 47 C.F.R.§54.201(a)(2).
5.I also certify that all federal universal service support funds received by Albion Telephone Company,
Inc.during the current calendar year will be used in a manner consistent with section 254(e),that is,for
the provision,maintenance,and upgrading of facilities and services for which the support is intended.The company will continue to comply for the period of January 1,2023,through December 31,2023,to
be eligible for federal universal service fund support.
6.This verification and affidavit is provided to be the Idaho Public Utilities Commission to enable the IPUC
to certify to the FCC that federal universal service support received by the eligible carriers in the statewillbeusedinamannerconsistentwS)of the Teleœmr unic ions Act

situations and has the following capabilities:Back-up power is provided to ILEC central offices by use of

SONET,DWDM,and MPLS technology deployed in its core fiber optic network that is self-healingand

core network.ILEC also has proper staff in place to repair any fiber cuts in a timely manner.ILEC has
connectivity with neighboring telephone exchanges as well as the LATA tandem to provide diverse
options to reroute traffic should an emergencyarise.ILEC has developed and trained its staff on

As of January 1,2023,the highest rate charged by Albion Telephone Company,Inc.("ATC")for

Residential customers of Albion Telephone Company,Inc.d/bla ATC Communications ("the Company")who qualify fortheLifelineProgramreceiveadiscountof$7.75 on local voice telephony service only ($5.25 federal discount +$2.50statediscount).
For all of the Company's exchanges,the Lifeline single-line rate for flat-rated local voice telephony service,including anymandatoryextendedareaservicechargeandthefederalsubscriberlinechargeis$24.51 ($32.26 standard rate -$7.75discount).All Customers of flat-rated local voice service,including Lifeline customers,have an unlimited number ofminutesforlocalcallsmadewithinthelocalcallingarea.
In addition,Lifeline customers may elect to subscribe to measured local voice telephony service at a rate of $16.75(24.50 standard rate -$7.75 discount).All customers of measured local voice service,including Lifeline customers,have an unlimited number of minutes for incoming calls and 90 minutes for outgoing calls made within the local callingarea.Customers pay $.03/minute for any minutes that exceed 90 minutes of outgoing calls.
For calls outside of the local calling area the customer can subscribe to long distance service from the Company'saffiliate,ATC Long Distance.Three plans are offered:(1)$.12/minute for in-state calls,$.05/minute for out-of-state calls,and a $6.95/month fee;(2)$.12/minute for in-state calls,$.07/minute for out-of-state calls and a $4.95/month fee;(3)$.12/minute for in-state calls,$.10/minute for out-of-state calls,and no monthly fee..Customers may also chooseanotherlongdistancecarrier.Customers may elect to subscribe to toll blocking at no charge.
