The ldaho Pubtic Utillties Commission Order No. 29&41 requires that Eligible Telecommunicatims Caniers
(ETC) codiry that it ls compliant with applicable service quality standards and consumer protec{ion rules; and
ETCs must demonetrate he ablllty to remaln functlonal ln emergencies. ln addltion, the Commlsslon must file
an annual certification with the USAC and the FCC that allfederal h'rgh-cqst support provided to ETCs within
fre State of ldaho will be used only for the provision, maintenance, and upgnading of facllites and seruioes for
whlch the support is intended. Accordingly, the undersigned stiates and veriffes under oath the folloving:
