The ldaho Publlc Utilities Commlssion Order No. 29&41 requires that Ellglble Telecommunicatlons Caniers
(ETC) carilry that lt is compliant with appllcable service quality strandards and consumer protedion rules; and
ETGs must demonstnate the abllity to remain functional in emergencles. ln eddltlon, the Gommission must file
an annual certlflcation with the USAC and the FCC that allfuderal high-cost support provlded to ETCs within
the Sbte of ldaho will be used only br the provlslon, maintenance, and upgrading of facilitles and servicee for
which the support is intended. Accordingly, the underslgned states and vermes under oath the following:

universal service support under section 2'14(e) of the Telecommunications Act of 1996 in the shte of

service quality standards and consumer protedion rules as eet brth in Commlsslon Order No. 29&41.
