The ldaho Public Utilities Commission OrdEr No. 2984'l requires that Eligible Telecommunications Carriers(ET-c) certify that it is compliant with applicable service quatity standards ind consume, prote"tion rutes; andETCs must demonstrate the ability to remain lunctional in emergencies. ln addition, the bommission must filean annualcertification with the USAC and the FCC that all federal hlgh-cost support provided to ETCs withinthe State of ldaho will be used only for lhe provision, maintenance, ani upgrading of facilities and services forwhich the support is intended' Accordingly, the undersigned states ano verines under oath the following:
1' I am anofficer of Newmax LLC dba lntermax Networks, an eligible telecommunications carrier forreceiving federal universal servlce support under section 214(;) of the Telecommunications Act of'1996 in the state of ldaho.
2, I am familiar with theCompany's day-today operalions in he state of ldaho and with the State,s servicequality standards and consumer proteclion rules as set forth in Commission Order No. 29g4.
3. Newmax LLc dba lntermax Networks is complying with applicable service quality standards andconsumer protection rules of lhe Federal Communications Commission and the ldaho public
Utilities Commission.
4. I certify to the Commission that the Company is able to remain functional in emergencles as ser lorth inCommission Order No. 29841 and in 47 C.F.R. S 54.201(a)(2).
