AFFIDAVIT OF BUSINESS OR CORPORATE OFFICER
The ldaho Public Utilities Cornmi$ion Order No. 29841 requires that Eligible Teleommunkmtions Caniers(ETC) certi8 that it is compliant with applicabb service qual.ty standards and onsumer protec*ion rules; and
ETCs must demonsrate the ability lo remain functional in emergencies. ln addition, the Commission must file
an annual cerlification with the USAC and the FCC that all bderal high-oost support provkled lo ETCs within
the State of ldaho will be used only for the provision, mainlenance, and upgrading of facilities and services for
which the supporl is intended. Accordingly, the undersigned states and verifies under oath the following:
1. I am an ofier ol Fi{st Step lntgmet. LLC , an eligible tebcommunications canier for receiving
federal universal servioe support under seclion 214(e) of the Telecommunications Acl of 1996 in the
state of ldaho.
2. I am familiar with the Company's day-today operations in the state of ldaho and wiUr the State's service
quality stiandards and consumer protection rules as set forth in Commission Order No. 29841.
3. First Steo l"qtemg!. UC is complying witr applicaUe service qualig standards and @nsumer
protedion rules of the Federal Communicalions Commission and the ldaho Public Utilities Commission.
4. I certify to the Commission that the Company is able to rernain functional in ernergencies as set forth in
Commission Order No. 29E41 and in 47 C.F.R. g 54.202(aX2).
5. I also certify lhat all federal universal service supporl funds reoeived by Flrsl Step lntemet, LLC*
during the cunent calendar year will be used in a manner consistent withsedion 2SllgLthaUs, for the
provision, maintenance, and upgr:ading of facilities and services br wtricfi the support is intended. The
company will continue to comply br the period of January 1.2021, through December 31. 2021, to be
eligible fur federal universal service fund supporl.
6. This verification and affidavit is ptovided to be the ldaho Public Utilities Commission to enable the IPUC
lo certify to the FCC that bderaluniversal service support received by the eligible caniers in the stete
will be used in a manner consistent with Sestion 2il(e) of the Telecommunications Acl.
