The ldaho Public Utilities Commission Order No. 29841requires that Eligible Telecommunications Carrierscertify.that it is compliant with applicable service quality standards and Jonsumer protection rules; and ETCsmust demonstrate the ability to remain functional in emergencies. ln addition, the Commission must file anannual certification with the USAC and the FCC that all federal high+ost support provided to ETCs within theState of ldaho will be used. onlyfor the provision, maintenance, aid upgradihg of iacilities and services forwhich the support is intended. Accordingly, the undersigned states anJ verifiEs under oath the following:
1. I am an officer of Pirect Communications Rockland , an eligible telecommunications carrier forreceiving federal universal service support under section 214(e) of the Telecommunications Act of 1gg6in the state of ldaho.
2. I am familiar with the Company's day{o-day operations in the state of ldaho and with the State,sservice quality standards and consumer protection rules as set forth in Commission Order No. 2g841.
3' Direct Comrnunications-Bocl-<land . is complying with applicable service quality standards andconsumer protection rules of the Federal Communicationi Commission and the ldaho fublic UtilifiesCommission.
4. l_certify to the Commission that the Company is able to remain functional in emergencies as set forth inCommission Order No. 29841 and in 47 C.F.R. g 54.201(a)(2).
5. I also. certify that all federal universal service support funds received by Direct Communications
Bochla'ld,duringthecurrentca]endaryearwitibeusedinamanneicoffi(e);that is, f9r !!e provision, maintenance, and upgrading of facilities and services for which the support isintended. The company will continue to comply for the period of January 1, ZOZO, through December31,2020, to be eligible for federal universal service fund support
6. This verification and affidavit is provided to be the ldaho public Utilities Commission to enable the IPUCto certify to the FCC that federal universal service support received by the eligible carriers in the statewillbe used in consistent with Section 25a(e) of the Telecommun Act.
