Frontier Communications Northwest Inc. ("FC Northwest") hereby files a state affidavit. FC Northwest
confirms that it successfully filed and certified Program Year 2020 FCC Form 481 and required attachments
with USAC on June 14,2019.
FC Northwest requests the Idaho Public Utilities Commission to file the annual certification regarding federal high-
cost support with the Universal Service Administrative Company (USAC) and the FCC, pursuant to 47 CFR

The ldaho Public Utilities Commission Ordar No. 29841 requires that Eligible Telecommunications Caniers (ETC)
cerlify that it is compliant with applicabJe service quality standards and consumer protection rules; and ETCs must
demonslrate the abillly to rernain functionalin emergencies. ln addition, the Gommissir:n must lile an annual
cefiification with the USAC and the FCC that allfederalhigh-cost support provided to ETGs within the Slate of
ldaho will be used only for the provision, maintenance, and upgrading ol facililies and services for which the
support is intended. Accordingly, the undersigned states and verilies under oath the lollowing:
1. I am an oflicer of Frontier Communications Northwest Inc., an eligible telecommunications carrier for
receiving federal universal service support under section 214(e) of the Te[ecommunications Act ol 1996 in
the state of ldaho.
2. I am familiar with the Company's day-to-day operations in the state of ldaho and with the State's service
qualily standards and consumer protection rules as set lorlh in Commission Order No. 29841.
3. Frontier Communications Northwest lnc, is complying with applicahle service quality standards and
consumer protection rules of the FederalComrnunications Commission and the ldaho Public Utilities
Commission.
4. I certify to the Commission that the Company is able to remain lunctional in emergencies as sel forth in
Commission Order No. 2984'l and in 47 C.F.R. $ 54.201(aX2).
5. I also certify that all federal universal service support funds received by Frontier CoF.munications
Northwest lqg during the current calendar year will be used in a manner consistent with section 25a(e);
that is, for the provision, mainlenance, and upgrading of lacilities and services lor which the support is
intended. The company will conlinue to comply for the period of January 1 ,2A2O, through December 31,
2020, to be eligible for federal universat service lund support.
6. This verification and allidavil is provided to be the ldaho Public Utilities Commission lo enable the IPUC
to certily to the FCC that federal universal service support received by the eligible carriers in the state will
be used in a manner consistent with Section 25a(e) ol the Telecommunications Act.
