Citizens Telecommunications Company of Idaho ("CTC Idaho") hereby files a state affidavit. CTC Idaho
confirms that it successfully filed and certified Program Year 2020 FCC Form 481 and required attachments
with USAC on June 14,2019.
CTC Idaho requests the Idaho Public Utilities Commission to file the annual certification regarding federal high-cost

OF COMPLIANCE WITH SERVICE QUALITY AND CUSTOMEB
PROTECTION, ABILITYTO REMAIN FUNCTIONAL IN EMERGENCIES,
AND USE OF FEDERAL HIGH-COST SUPPORT.

The ldaho Public Utilities Commission Order No. 29841 requires that Eligible Telecommunications Carriers (ETC)
certify that it is compliant with applicable service quality standards and consumer protection rules; and ETCs must
demonstrate the ability to remain lunctional in emergencies. ln addition, the Commission must file an annual
certilication with the USAC and the FCC that alllederalhigh-cost support provided to ETCs within the State of
ldaho will be used only for the provision, maintenance, and upgrading ol lacilities and services lor whhh the
support is intended. Accordingly, the undersigned states and verilies under oath the following:
1. I am an officer ol Gitizens Telecommunications Comoanv of ldaho. an eligible telecommunications carrier
for receiving federal universal service support under section 214(e) of the Telecommunications Act of
1996 in the slate of ldaho.
2. I am lamiliar with the Compant's day-to-day operations in lhe state of ldaho and with the State's service
quality slandards and consumer proteclion rules as set loih in Commission Order No. 29841.
3. Citizens Telecommunicatigns Comoanv of ldaho is complying with applicable service quality slandards
and consumer proteclion rules of the Federal Communications Commission and the ldaho Public Utilities
Commission.
4. I cerlify to the Commission that lhe Company is able to remain funclional in emergencies as set forth in
Commission Order No. 29841 and in 47 C.F.R. $ 54.201(aX2).
5. I also certify that all federal universal service support funds received by Citizens Telecommunicatiq0s
Comoanv of ldaho during the current calendar year will be used in a manner consistent with section
254(e); that is, for the provision, maintenance, and upgrading ol facilities and services for which the
support is intended. The company will continue to comply lor the period ol January 1,2O2O, through
December 31, 2020, lo be eligible for lederal universal service fund support.
6. This verification and affidavit is provided to be the ldaho Public Utilities Commission lo enable the IPUC
to certily to the FCC that federal universal service support received by the eligible caniers in the stale will
be used in a manner consistent with Seclion 254(e) of the Telecommunications Act.
