Dear Ms. Hanian:
On January 30,2019, Citizens Telecommunications Company of Idaho d/b/a Frontier Communications of
Idaho (Study Area Code 474427) and Frontier Communications Northwest, lnc. (Study Area Code 472416)
herein after referred to as "Frontier" submitted a copy of its Annual Lifeline Eligible Telecommunications
Carrier Certification FCC Form 555 in compliance with 47 CFR 54.416 as adopted by the Federal
Communication Commission (FCC) in its Lifeline Reform Order, FCC l2-ll, released February 6,2012.
Frontier respectfully requests that the previously submitted copy be replaced with the attached REVISED copy.
The list of affiliates on page 6 of the FCC Form 555 was inadvertantly excluded from the previously submitted
copy.
