On June 21, 2017 Frontier Communications Corporation (“Frontier”) submitted to the Federal Communications
Commission, pursuant to 47 C.F.R. § 54.312 (c) (4) the enclosed list of census blocks that were previously
identified in its election of Connect America Fund (“CAF”) Phase I Round 2 incremental support to which
Frontier did not deploy broadband using Round 2 support solely due to changes in Frontier’s deployment plans.
The list does not reflect census blocks that Frontier previously identified for Round 2 deployment but were
removed from Round 2 deployment due to challenge process determinations or assertions that the census blocks
were already served.
Frontier hereby provides the Idaho Public Utilities Commission a copy of that information. If you have any

Dear Ms. Dortch:

Frontier Communications Corp. (“Frontier”), pursuant to 47 C.F.R. § 54.312(c)(4), is submitting the
attached list of census blocks for which we previously had identified locations to which we would
deploy broadband service using Connect America Fund (“CAF”) Phase I Round 2 incremental
support but for which we now know we will not be deploying broadband service using that
support. Frontier initially accepted CAF Phase I Round 2 support on February 24, 2014,1 and
subsequently provided updated deployment information on March 3, 2014,2 February 24, 2015,3
August 26, 2016,4 and December 8, 2016.5 In the intervening months, Frontier has refined its
broadband deployment plans and determined that the census blocks in the attached list are no
longer part of our CAF Phase I Round 2 deployment plans.

Please note that the attached list only reflects the previously-identified Round 2-eligible census
blocks to which Frontier did not deploy broadband using Round 2 support solely due to changes in
