Co.'s (collectively referred to as "Blackfoot") 2016 FCC Form 481s, submitted to the
Universal Service Administration Corporation and the Federal Communications
Commission pursuantto 47 CFR 554.313 and $54.422; and 2) Affidavits supporting
designation as an Eligible Telecommunications Carrier.
As part of its FCC Form 481, Blackfoot is submitting its progress on its five-year network
build-out plan as well as detailed financial data. These items are trade secrets and
proprietary pursuant to ldaho Code S 9-340D and are exempt from pubtic disclosure.
Accordingly, Blackfoot has sealed these in the enclosed envelopes and printed them on
yellow paper.

PROTECTION, ABILITY TO REMAIN FUNCTIONAL IN EMERGENCIES,
AND USE OF FEDERAL HIGH-COST SUPPORT.
AFFIDAVIT OF BUSINESS OR CORPORATE OFFICER
The ldaho Public Utilities Commission Order No. 29841requires that Eligible Telecommunications
Carriers (ETC) certify that it is compliant with applicable service quality standards and consumer
protection rules; and ETCs must demonstrate the ability to remain functional in emergencies. ln addition,
the Commission must file an annual certification with the USAC and the FCC that all federal high-cost
support provided to ETCs within the State of ldaho will be used only for the provision, maintenance, and
upgrading of facilities and services for which the support is intended. Accordingly, the undersigned states
and verifies under oath the following:
1. I am an officer of Blackfoot Telephone Cooperative, lnc., an eligible telecommunications carrier for
receiving federal universal service support under section 214(e) of the Telecommunications Act of 1996 in
the state of ldaho.
2.1 am familiar with the Company's day{o-day operations in the state of ldaho and with the State's
service quality standards and consumer protection rules as set forth in Commission Order No. 29841.
3. Blackfoot Telephone Cooperative, lnc. is complying with applicable service quality standards and
consumer protection rules of the Federal Communications Commission and the ldaho Public Utilities
Commission.
4. I certify to the Commission that the Company is able to remain functional in emergencies as set forth in
Commission Order No. 29841 and in 47 C.F.R. $ 54.201(a)(2).
5. I also certify that all federal universal service support funds received by Blackfoot Telephone
Cooperative, lnc. during the current calendar year will be used in a manner consistent with section
25a@); that is, for the provision, maintenance, and upgrading of facilities and services for which the
support is intended. The company will continue to comply for the period of January 1, 2017 , through
December 31,2017, to be eligible for federal universal service fund support.
6. This verification and affidavit is provided to be the ldaho Public Utilities Commission to enable the
IPUC to certify to the FCC that federal universal service support received by the eligible carriers in the

customer requests for broadband service that were unfulfilled in the prior calendar year.
Blackfoot Telephone Cooperative, lnc. (482235,483308) and Fremont Telecom Co. dba
Fremont Communications (472222) routinely replace digital loop carriers that are not
capable of providing a minimum of 4 Mbps download and 1 Mbps upload to new loop
carriers and provide Ethernet transport, where appropriate. Additionally, the companies
deploy broadband accelerators in line that extend DSL or increase the speed to
customers whose requests would othenryise be unfulfilled.

service quality standards and consumer protection policies and procedures. This document
provides a high level description of the measures in place.
Service Qualitv
The companies comply with service quality standards by meeting all requirements in the
Administrative Rules of Montana 38.5.3371and ldaho Administrative Code l.D.A.P.A.
31.41.01.500, as well as, all applicable federal consumer protection rules.
Consumer Protection
The companies comply with consumer protection obligations by meeting requirements in
Administrative Code l.D.A.P.A. 3t.41-.0t, applicable consumer protection regulation in the state
of Montana, as well as, all federal consumer protection rules. The companies'general practice
is to treat all information as if it were private, in addition CPNI and Red Flag policies and
procedures are observed and trained on annually. CALEA obligations and processes are strictly
adhered to, Noticing is undertaken annually in relation to do not call and call before you dig.
Noticing of rate changes is provided to educate customers about changes appearing on their
bills regardless of the jurisdiction or regulated nature of the service.
Functionality in Emergency Situations

This document provides a high level description of the measures in place to provide
functionality in Emergency situations in the 482235, 483308 and 472222 study areas.
Central Offices in all 3 study areas are equipped with backup generators in the event of
commercial AC power failures. Fuel supplies for these generators are adequate for 24-
36 hours of operation and back up batteries located in each central office provide an
additional 6-8 hours of backup power.
All remote subscriber carrier locations are equipped with backup batteries that are
capable of providing 8-12 hours of DC power in the event of a commercial AC power
failure. We also maintain a pool of portable generators that are used to recharge these
batteries if the AC power is not restored prior to the batteries being fully discharged.
Where practical, fiber optic cable routes that provide connectivity to a remote central
office back to the host central office have diverse routes to insure uninterrupted
operation in the event of a cable cut or failure. lf diverse routes are not practical the
remote central office is equipped with a "stand alone" function that insures uninterrupted
operation within the remoter central office service area.

Blackfoot Telephone Cooperative, lnc. ("BTC") provides a broad range of services to the
Confederated Salish Kootenai Tribe ("CSKT') on the Flathead lndian Reservation in
northwestern Montana. ln fact, the CSKT is one of BTC's largest customers.
Over the past year, BTC worked with CSKT on a number of projects/initiatives and we
look fonruard to continuing our partnership. The following is a timeline of events with
summaries of our engagement activities:
ln April 2015 Blackfoot delivered the CSKT's Capital Credit Checks for patronage
distributed to the CSKT as a member of Blackfoot Telephone Cooperative, lnc.
During the months of May, June and July Blackfoot worked with CSKT to turn down
circuits that they no longer needed, updating billing information and authorized contacts.
ln August of 2015, Blackfoot met with key CSKT personnel. An onsite visit was held
with the CSKT's Phone Network Manager to review their account and discuss additional
service offerings. Additional voice services were requested as a result of that meeting
to certain CSKT buildings.
ln September of 2015, Blackfoot met with the CSKT's lT Network Administrator and lT
Manager to review the account and discuss additional service offerings to include
redundancy/failover, colocation facilities and disaster recovery.
ln October, new services were installed but much of what had been discussed during
the September meeting was on hold while CSKT filled a new lT Program Manager
position. ln December, we were notified that the position had been filled so a proposal
was submitted in January of 2016.
Blackfoot is in compliance with all tribal rights of way processes, land use permitting,
facilities siting rules, environmental review processes, cultural preservation review and
business licensing requirements
Voice Services Rate Comparability

For the Program year 2017, the average urban rate for local service is $21.93. Per FCC
Public Notice DA 16-362 the reasonable comparability benchmark for voice services is
$41.07 . As indicated in each study area's line 700 worksheet none of the Blackfoot
rates are above the reasonable comparable rate of $41.07.

voice/broadband bundles at different prices. To ensure compliance with the FCC's rate
comparability benchmark for broadband services, Blackfoot offers a distinct set of
broadband service offerings that are priced at rates that are equal to or less than the
FCC's service rate comparability benchmark. These prices are revised annually as the
FCC adjusts the prices, if necessary, to ensure they remain equal to or less than the
rate comparability benchmark.

For the Program year 2017, Lifeline consumers have the same voice and data options
available to them as non-lifeline consumers. Rates can be reviewed in sections 700
and 710 of the 481 filing. The applicable terms and conditions are the same for any
consumer regardless of low income determination. Local voice services are unlimited
as are data services. Consumers wishing to purchase long distance services have a
selection of service options to ensure that the per minute pricing meets their needs.
Eligible lifeline consumers have a lifeline discount added to their account after eligibility
has been determined.
