Enclosed for filing with Commission is the Eligible Telecommunications Carrier (ETC)
Annual Recertification for Qwest CorporationdlblaCenturylink QC (Southem Idaho).
Qwest Corporation dba CenturyLink QC (Southern Idaho) ("Centurylink") hereby
requests that the Public Utilities Commission of Idaho submit a letter to the Federal
Communications Commission ("FCC") and the Universal Service Administration Company
("USAC") prior to October 1,2016, verifying that CenturyLink is in compliance with section
$25a(e) of the Telecommunications Act and notifying them that CenturyLink is eligible to
receive federal universal service support in accordance with $25a(e) of the Telecommunications
Act and 47 C.F.R. $54.314. Attached for filing with the FCC and USAC is an affidavit attesting
to the fact that any Federal Universal Service Funds are utilized as intended in compliance with
the Telecommunications Act.

The ldaho Public Utilities Commission Order No. 29841 requires that Eligible Telecommunications Carriers
(ETCs) certify that they are compliant with applicable service quality standards and consumer protection rules;
and ETCs must demonstrate the ability to remain functional in emergencies. ln addition, the Commission must
file an annualcertification with the UniversalService Administrative Company and the FederalCommunications
Commission (FCC) that all federal high-cost support provided to ETCs within the State of ldaho will be used only
for the provision, maintenance, and upgrading of facilities and services for which the support is intended.
Accordingly, the undersigned states and verifies under oath the following:
1. I am an officer of Qwest Corporation d/b/a CenturyLink QC (Southern ldaho). an eligible
telecommunications carrier receiving federal universal service support under section 214(e) of the
Telecommunications Act of 1996 in the state of ldaho.
2. I am familiar with the Company's day-today operations in the state of ldaho and with the State's service
quality standards and consumer protection rules as set forth in Commission Order No. 29841.
3. Qwest Corooration d/b/a CenturyLink QC (Southern ldaho). is substantially complying with applicable
service quality standards and consumer protection rules of the Federal Communications Commission and
the ldaho Public Utilities Commission.
4. I certify to the Commission that the Company is able to remain functional in emergencies as set forth in
Commission Order No. 29841 and in 47 C.F.R. S 54.201(aX2).
5. I also certify that all federal universal service support funds received by Qwest Corooration d/b/a
CenturvLink QC (Southern ldaho) during the current calendar year will be used in a manner consistent
with section 25a@); that is, for the provision, maintenance, and upgrading of facilities and services for
which the support is intended. The company will continue to comply for the period of January 1,2017
through December 31,2017, to be eligible for federal universal service fund support. CenturyLink is
certifying its compliance with 47 C.F.R. 54.313(c) at a holding company level, as allowed per the
provisions of DA 13-2101 released October 30, 2013.
6. This verification and affidavit is provided to be the ldaho Public Utilities Commission to enable the IPUC
to certify to the FCC that federal universal service support received by the eligible carriers in the state will
be used in a manner consistent with Section 254(e) of the Telecommunications Act.

As part of its efforts to strengthen positive relationships with the Tribal Nations within its
serving territory while fulfilling obligations set forth in Report and Order and Further Notice of
Proposed Rulemaking, FCC L1-!61, Qwest Corporation dlbla Centurylink QC (Southern ldaho)
("CenturyLink")extended an invitation to the'above listed tribal entity to meet and address issues
of importance related to the provisioning of services on tribal lands. The purpose of these
meetings was to discuss planning and potential deployment of service as well as other areas
of interest specific to conducting business on tribal lands (as set forth in 47 C.F.R. 554.313(aX9))
which at a minimum, would include:
(i) A needs assessment and deployment planning with a focus on Tribal community anchor
institutions;
(ii) Feasibi lity and sustaina bility planning;
(iii) Marketing services in a culturally sensitive manner;
(iv) Rights of way processes, land use permitting, facilities siting, environmental and cultural
preservation review processes; and
(v) Compliance with Tribal business and licensing requirements. Tribal business and licensing
requirements include business practice licenses that Tribal and non-Tribal business entities,
whether located on or off Tribal lands, must obtain upon application to the relevant Tribal
government office or division to conduct any business or trade, or deliver any goods or
services to the Tribes, Tribal members, or Tribal lands. These include certificates of public
convenience and necessity, Tribal business licenses, master licenses, and other related forms
of Tribal government licensure.

the critical role communieations services play in elilninating the digital divide and
improving the economic, health and educational opportunities for CenturyLink's
c,ustomers living on Tribal lands.
h 2011, the Federal Comrnunications Commission (*FCC") modernimd the
communications landscape by implementing steps to ensure that robusf affordable voice
and broadband service are available to Americans throughout the nation iucluding those
residing on Tribal lands.l In furthering CenturyLink's goal of building a cooperative
relationship with the Shoshone-Bannock Tribes, and the FCC's goal of comprehensive
engagement $rith Tribal govemments r*garding communications services,I would like to

<1210> Terms & Conditions of Voice Telephony Lifeline Plans
CenturyLink, lnc. has over 100 local exchange carriers (LECs) that serve as eligible
telecommunications carriers (ETCs) providing Lifeline discounts on localtelephone service for
qualifying low-income customers in thirty-seven states. Each LEC's tariff or local terms of
service contain the terms and conditions of voice telephony service plans generally available to
CenturyLink residentialcustomers. Lifeline provides discounts on CenturyLink residential
service plans that include voice telephony service. Lifeline discounts provided to qualified
recipients include the $9.25 per month federal discount plus state discounts, if available. Tribal
Lifeline recipients receive an additionalfederal Lifeline discount of up to $25 per month. Eligible
residents residing on Tribal lands in areas where CenturyLink receives universal service high-
cost support can also receive a Tribal Link Up credit of not more than $100 against one
customary service initiation fee at a primary residence.
CenturyLink's flat-rated residential service plans provide unlimited local calling. Lifeline
discounts also may be applied to local residential service plans that include a certain amount of
local minutes or calls at a flat-rate and then have additional charges for minutes or calls beyond
those included in the plan. Lifeline discounts may be applied to bundled service packages that
include voice telephony services, such as bundles with internet service and/or video service.
Lifeline discounts may also apply to voice service plans that include optional calling features
such as caller lD, callwaiting, and voicemail.
Toll service is available to customers receiving Lifeline discounts in the same manner that it is
available to non-Lifeline customers. Toll limitation service is available to Lifeline customers at
no charge.

B. (7 77) CenturyLink is reporting a-la-cart and data-only broadband rates that meet or exceed both
the download speed of four mega bits per second and upload speed of one mega bit per second.
Upload speeds of 768K or below are not included in this report.
9. (800), (810)-(813), (1200), and (1210)-(L223) CenturyLink is a designated eligible
telecommunications carrier that receives Universal Service Support for High Cost Areas under 47
C.F.R. 54 subpart D. Therefore, Centurylink is only subject to subpart (a) of 47 C,F.R. 554.422 as it
applies to this filing.
10. (810) - (813) Per FCC DAL3-1707 released August 6,2013, CenturyLink is reporting holding
company, operating companies, and affiliates (as defined under section 3 of the Communications
Act of 7934, as amended, 47 U.S.C. S 153(2)) that are designated as eligible telecommunications
carriers and/or that provide retail broadband internet access to end-user customers.
77. (927) - (929) To the extent the carrier serves federally recognized tribal lands, the attached
narrative, in response to line 920, should be relied on to describe tribal outreach and interaction. If
the carrier at least offered to discuss the points listed in 47 C.F.R. $5a.313(a)(9) to the federally
recognized tribes served in a study area, lines 927-929 were marked with a "yes" response.
12. (1000) - (1010) Carriers must certify that their local rates are at or below two standard
deviations of the applicable national average urban rate for voice service, as specified in the most
recent public notice issued by the Wireline Competition Bureau and Wireless Telecommunications
Bureau. Qwest Corporation d/b/a CenturyLink (WyomingJ and United Telephone Company of the
West d/b/a Centurylink fWyoming) have certain exchanges with explicit cost based local rates
above this standard. Customers in such exchanges receive a Federal Universal Service credit and/or
a State Universal Service credit explicitly on their bill. This results in a net charge that is lower than
two standard deviations ofthe applicable national average urban rate forvoice service.
13. (2010), (2077), (2015) and (2016) Per FCC DA 13-2101 released October 30,2073, Centurylink
is certifying at a holding company level.
14. (2017) and (2025) Centurylink, in the November 9,2015 ln the Matter of ETC Annual Reports
and Certificatrons; WC Docket No. 14-58, provided location data for Connect America Fund Phase 1
Incremental Support Program Round 1 ("Round 1")for both the second and third year completion
periods. Therefore no Round 1 data is provided in this filing. Future network utilization could
impact broadband service at the previously disclosed locations.
