Dear Ms. Jewell:
Eligible Telecommunications Carriers ("ETCs") who receive federal high cost or low income support are
required to file annual reports ("Annual ETC Reports") pursuant to 47 C.F.R. $5 54.313 and 54.422 with
the Federal Communications Commission ("FCC"), the Universal Service Administrative Company
("USAC"), and the relevant state commissions or Tribal governments, as appropriate. See 47 C.F.R. S
54.313(i); 5 5a.a22ft1. The Annual ETC Reports are required to be filed on FCC Form 481, with attached
materials.
In fulfillment of this requirement, Potlatch Telephone Company, (the "Company'), has enclosed a
confidential and redacted copy of its FCC Form 481, along with the Request for Confidential Treatment
letter dated lune 27,2016 to Marlene H. Dortch, Secretary, Federal Communications Commission.
Pursuant to the Code of Federal Regulations cited in the FCC letter and IDAPA 31.01.01, Rule 57,
Company requests that certain portions of its FCC Form 481 as outlined in the FCC letter be treated as
confidential information and not subject to public inspection.
Company respectfully requests that the ldaho Public Utilities Commission afford this information the
same confidential treatment that has been sought from the FCC as public disclosure would cause
significant commercial and competitive harm to the Company.

As an lncumbent Local Exchange Carrier (ILEC) and Eligible Telecommunications Carrier (ETC), Potlatch
Telephone Company has been providing ubiquitous, high-quality voice telecommunications services in
its study area for many years. To accomplish and maintain this service level, Potlatch has made
significant historical investment to deploy, operate, and maintain an integrated, highly-reliable network.
ln addition to its own capital spending, Potlatch draws from the federal Universal Service Fund (USF).
Universal service support has been (and continues to be) critical in enabling Potlatch's services in its
rural markets to be reasonably comparable in quality and price to services in more urban markets, as
Congress mandated in the Telecommunications Act. Potlatch draws USF support because the cost of
providing voice and data services in its rural study area are substantially higher than those in urban
areas, and thus all of the costs cannot be recovered solely from Potlatch's customers while maintaining
reasonably comparable prices. Potlatch has made investments to bring high speed data services to its
customers when the level of customer revenues and universal service support has made it financially
viable to do so.
For Potlatch, federal high cost support is used to help offset ongoing network costs, but the monies
received cover only a portion of the cost of updating and operating the network. ln 2015, Potlatch
received S369,997 in USF support while incurrtntlin operating expenses and investing
I'r new plant. The attached Schedule A contains a list of specific network improvement projects
that were completed in 2015 at the wire center level. Where these projects related to specific DSAs
within the wire center, it is so indicated and can be cross-referenced to the exchange map attached as
Exhibit 1. The attached Schedule B summarizes Potlatch's actual expenses for 2015. As evidenced by
these support and expenditure numbers provided for the current reporting year, the universal service
support that Potlatch receives covers only a fraction of its cost to provide service. Continued receipt of
USF support is vital to helping Potlatch maintain reasonably comparable rates for local exchange service;
and to incrementally upgrade its telecommunications facilities and equipment to help meet evolving
service requirements and maintain high quality service.

Carrier has implemented service availability tracking tools and employee training capabilities to respond
to direct customer requests for broadband services.
Upon receipt of a new broadband service request, Grrier's service advisors follow these steps for
provisioning the service:
1) The C-arrierservice advisor uses a customized service addressability softwaretool todetermine
if broadband service is available to the requested service address. lf it is determined that
service is offered to the address, an installation order will be initiated and scheduled
immediately.
2l lf the information in the service addressability tool indicates that extension of broadband
service to the service address might be possible, a field service technician is dispatched to the
customer premise to perform additional diagnostic testing. Such testing will determine whether
there are any reasonable adjustments to the network or customer facilities which ca n be made
toenablethe provision of service. lf testsconfirm thatbroadband servicecanbe offeredatthe
service address, an order is initiated and service is provisioned.
3) ln situations where C:rrier'sterrestrialbroadband service is not available to a requesting
customer, Carrier has partnered with Dish Networkto offer dishNET satellite broadband service
to customers. C:rrier'sservice advisors are trainedto discuss and assist the customer in
ordering dishNET broadba nd service.
As the Commission acknowledgedl, some of the service areas served by rate of return carriers, like
Carrier, have characteristicsthat make it highly cost prohibitive to extend broadband service using
terrestrialwireline technology. Except as may be noted in Carrier's 5-year pla n attached to this filing,
any further build-out of terrestrial broadba nd service to additiona I locations within its study a rea will be
dependentuponthecostofthetechnologytobedeployedandthe capitalinfrastructurefundinglevel
available. After following the steps above, if the service request is still unfulfilled, it is due to the
customer'sdistancefromabroadbandcapablecentralofficeorDSA. Thecapitalcosttodeployfiber
and other equipment to shorten loop lengths to support broadband at the required levels well exceeds

TDS Telecommunications Corporation's ILEC companies follow applicable federal and state
service quality and consumer protection rules. They comply with quality of service
requirements including monitoring and reporting service quality metrics where required. TDS
Telecom has implemented numerous consumer protection measures to protect customer
information. For example, TDS implemented Customer Proprietary Network lnformation (CPNI)
policies and procedures that are consistent with the FCC's regulations. Employees are required
to complete CPNI training and in addition, employees who have access to CPNI data receive
additional guidance through written procedures regarding customer authentication. Annually,
all employees are required to review TDS' Business Code of Conduct which includes information
and requirements on protecting sensitive customer information from improper use and
disclosure. TDS data privacy and security policies are reinforced through periodic training
required of all employees. Additional consumer protection measures include TDS' use of a
third-party verifier to prevent unauthorized presubscribed interexchange carrier (PlC) changes
("Slamming") and the elimination of billing and collection arrangements that could have
potentially allowed unauthorized third-party charges to be added to customer's bills
("Cramming").

Company is ableto remain functional in an emergencysituation throughthe use of back-up power to
ensure functionality without an externalpower source. The Company's standardfor batterybackup is 8

annual routine maintenance which includes batteryinspection, cleaning, documentation of float voltage
and cell temperature, aswell as equalization or replacement if necessary. ln addition, permanent

Potlatch Telephone servesthe Nez Perce Tribe in its Kendrick exchange in ldaho. During the
meeting, the participants discussed subjectswhich have been categorized below usingthe FCC
rule and guidance pubtished bythe Office of Native Affairs and Policyl regarding specifictopics
that should be covered in these meetings.
General
Danae mentioned there is a new tribal Chairman named Anthony Johnson. All official correspondence
including the Form 481s should be directed to Anthony.
Needs Assessment and Deplovment Planning
There was discussion of the Broadband Stimulus project that was completed in March 2014.
Danae asked some questions about the map that was included as part of the July 2015, 481 submission
to the FCC. Specifically, census block groups 327OL and 32707 are located on tribal lands. Per the map,
CBG 32707 indicates we have a proposed DSL site but there is no DSL today. However, CBG 32701
indicatesthereisDSLattheDSA,howeverthereisnoDSLavailable. (TDSneedstoreviewthemapand
correct as needed.) Randy explained we do have DSL available in the Arrow Junction area via Hubbard
Gulch.
The tribedoesn't have plansto add any newanchor institutionswiththeTDS servingarea.
However, the tribe is trying to get Bonneville power (BPA) to serve the fish hatchery with broadband.
Randy reported that we ca n't support broadba nd down there at this time and we have no current plans
todoso-thisisinthe3zT0lblockonthemap. DanaeandChristinasaidtheywill continuetopursue
this with BPA.

Danae described the tribe's efforts to train residents and senior citizens on effectively usingthe
computer and internet. They have training sessions on regular basis toteach residents how to use
computers (meet with senior citizens) including how to access email, the web, and procurement. They
also provide individual training with tribal members who call in or work with them through social
services.
Feasibilitv and Sustainabi! itv Plannine
Update on thetribal ownedwireless broadband system: The 2015 wireless growthcamewith a
new tower located outside the TDSserving area. They also received some Homeland Security Funds
thatwill be used to purchase backup generatorsat all of the towersites. The tribe also applied for
grantsthrough the RuralBroadband Experiment program which they were not awarded. No parties
received funding on tribal lands.
Marketing Services in a culturallv sensitive manner
The tribe continues to be heavilyinvolvedwiththe Lifeline program by educatingresidents,
especially senior citizens about the benefits of the Lifeline program.
ln February 2014 TDS sent brochures that included a map of the TDS service area. The tribe
prints copies of the brochures for the general council meetings to hand out to interested members.

Lifeline Service Overview
1. A Lifeline customer may subscribe to any local service offering available to all
residential customers. Such local service offerings include:
o Basic local exchange telephone flat rate service (R1) wtrich entitles the
customer to an unlimited number of telephone calls within the exchange and
local calling area (including EAS points) without additional charges. Toll
charges do not apply.
. A local measured service (if offered by the Company). The service is priced
louer than R1 service but either a per minute or per message rate also applies.
The measured service plan may include an allouance of minutes/messages.
o An expanded local calling service which is priced higher than R1 service but
includes a larger local calling service area without incurring toll charges.
o Any bundle service that includes residential basic local exchange service.

SECURITY LINE SERVICE
1. General
Security Line Service is a bundle for residential custorners who have their main
telecommunications service with a wireless provider but require a basic access line
for back-up and high speed data.
The bundle includes a Residential One-Party Line and up to 1 Mbps high speed
data. (Could be an LMS line at companies that provide LMS)
2. Terms and Conditions
a. Security Line Service will be provisioned where facilities are available.
b. Rules, regulations, and limitations as specified elsewhere in the Company's
tariffs will apply to this bundle.
c. ltlo other optional services or features are alloued with this bundle, except

Certification of Reasonable Steps Taken to Provide l0 x I Boadband Service

As part of the FCC's annualreporting requirements for ETC re-certification, Rule
54.313(0( I )(i) requires carriers to certify that upon receiving a customer's reasonable request for
broadband service with speeds of l0 mbps download and I mbps upload, the carrier took reasonable step
to provide service at those speeds.
TDS Telecommunications Corporation (TDS Telecom) owns and operates the tebphone
companies listed on the attachment. Each provides broadband service. Collectively through these
companies, TDS Telecom offers brmdband service to 94Yo of its ILEC customen. Most of these
customers receive speeds of 5 mbps or faster.
In 2015, TDS Telecom completed amulti-year initiative to extend its broadband
coverage to much of its ruralserving area. This effortutilzed grant funding available through the
American Recovery and Reinvesftnent Act and was administered by the Rurallhility Services (RUS).
$100 million in grant funding was received while TDS Telecom invested an additional $25 million of its
own capital At the conclusion of this initiative, broadband service was made available to an additional
32,000 customers who were previously unserved.
At this time, and with today's available technology, TDS Telecom has extended its
broadband footprint as far as is economicalty feasfole absent additional funding through universal service
support and/or other state and federal grant programs . The 60/o ofcustomers that remain unserved live in
our most ruraland remote service areas. The cost to bring broadband to them, or to increase speeds to l0
x I for those who currently have service at slower speeds, would far exceed the revenue received from 0re
sale of those services. We continue to closely monitor emerging technologies and look for affordable
ways to serve these custome$. As part of that effort, TDS Telecom is evaluating recent changes to the
universal service program for rate of return carriers to determine how the funds available going forward
can be utilized to efficiently extend broadband to additional customers in unserved areas and meet the
evolving speed requirements for customen in underserved areas.
