The FCC in its February 6, 2012 Order in WC Docket No. 11-42, required all state
designated ETC 's to provide, on an annual basis, the results of their Lifeline recertification
efforts to the applicable state commission.
Attached is the Annual Lifeline Eligible Telecommunications Carrier Certification Form,
Form 555, for AT&T Mobility which was filed with the FCC and USAC.
