AFFIDAVIT OF BUSINESS OR CORPORATE OFFICER
The Idaho Public Utilities Commission Order No. 29841 requires that Eligible Telecommunications Carriers certify
that it is compliant with applicable service quality standards and consumer protection rules; and ETCs must
demonstrate the ability to remain functional in emergencies. In addition, the Commission must tile an annual
certification with the USAC and the FCC that all federal high-cost support provided to ETCs within the State of
Idaho will be used only for the provision, maintenance, and upgrading of facilities and services for which the
support is intended. Accordingly, the undersigned states and verifies under oath the following:
1.1 am an officer of CTC Telecom, Inc., an eligible telecommunications carrier for receiving federal universal
service support under section 214(e) of the Telecommunications Act of 1996 in the state of Idaho.
2.I am familiar with the Company's day-to-day operations in the state of Idaho and with the State's service
quality standards and consumer protection rules as set forth in Commission Order No. 29841.
3.CTC Telecom, Inc., is complying with applicable service quality standards and consumer protection rules
of the Federal Communications Commission and the Idaho Public Utilities Commission.
4.I certify to the Commission that the Company is able to remain functional in emergencies as set forth in
Commission Order No. 29841 and in 47 C.F.R. § 54.201(a)(2).
5.I also certify that all federal universal service support funds received by CTC Telecom ,lnc., during the
current calendar year will be used in a manner consistent with section 254(e); that is, for the provision,
maintenance, and upgrading of facilities and services for which the support is intended. The company will
continue to comply for the period of January 1, 2013 through December 31, 2013, to be eligible for federal
universal service fund support.
6.This verification and affidavit is provided to be the Idaho Public Utilities Commission to enable the IPUC
to certify to the FCC that federal universal service support received by the eligible carriers In the state will
be used in a manner consistent with Section 254(e) of the Telecommunications Act.

Section 10: Two-Year Network Improvement Plan and Progress Report
The annual report must include a progress report on the canier's two-year service quality improvement plan,
including maps detailing its progress toward meeting the plan targets, an explanation of how much universal
service support was received and how it was used to improve signal quality, coverage, or capacity, and an
explanation regarding any network improvement targets that have not been fulfilled. This information shall be
submitted at the wire center level. The annual report must also include an updated two-year network
improvement plan indicating plans for future investment.
Two-Year Network Improvement Plan Format
Year 1: January 1, 2012—December 31, 2012*
Council Cell Site, Adams County - Conversion to Ethernet Transport w/increased data speeds
and upgrade cell site to EVDO Rev. A to include broadband data.
Expect to be finished with this project by October 1st 2012 at a cost of $60k.
Indian Valley Cell Site, Adams County - Conversion to Ethernet Transport speeds - November
2012- $15k
Squaw Butte/Montour Cell Site, Gem County - Continue feasibility studies with environmental
impact of cell site location - ongoing will continue into 213
Year 2: January 1, 2013—December 31, 2013*
Cambridge Cell Site, Washington County - Conversion to Ethernet Transport w/increased data
speeds and upgrade cell site to EVDO Rev. A to include broadband data.
Expect to be finished with this project by June 2013 at a cost of $60k.
Lowman Cell Site - Boise County - upgrade to EVDO rev. A. - November 2013- $45k
Squaw Butte/Montour Cell Site * Boise County - Install Base Station Controller - $55K
*This report is intended to be a forward-looking view of a carrier's network improvement plan.
Please do not provide information for any months earlier than those listed above.
Note: The ETC may wish to submit the Two-Year Network Improvement Plan as "confidential
information." If so, please follow the 1PUC Rules of Procedure for Information Exempt From
Public Review—Definitions—Form—procedures. See IIDAPA 31.01.01, Rule 67
