Dear Secretary:
As requested by the FCC in its USF/ICC Transformation Order (FCC 11-161) released November
18, 2011, its Clarification Order (DA 12-147) released February 3, 2012, and its Third Order on
Reconsideration (FCC 12-52) released May 14, 2012, companies seeking ETC certification were ordered
to file certain information and certifications with the FCC by July 2, 2012, and annually thereafter.
Furthermore, the FCC ordered companies to send a copy of their FCC ETC filing to their respective state
Commission. Accordingly, please find enclosed a copy of the filing that was made by TDS
Telecommunications Corporation's Idaho company.
