Dear Ms. Jewell:
Enclosed for filing on behalf of Fremont Telecom Co. dfb/a FairPoint Communications ("FairPoint") are
the following:
(1)Certification required in accordance with 47 C.F.R. §54.313(a)(5) and 54.313(a)(6), adopted by
the Federal Communications Commission in its Report and Order in WC Docket Nos. 10-90 et. al., FCC
No. 11-161 (November 18, 2011). These rules require recipients of high-cost support to provide: (a)(5)
"Certification that is complying with applicable service quality standards and consumer protection rules;"
and (a)(6) "Certification that the carrier is able to function in emergency situations as set forth in
§54.202(a)(2)." Attachment 1 provides the required certifications.
(2)In compliance with 47 C.F.R. §54.313(h), which requires recipients of high-cost support to
provide certain rate information that determines any applicable limitations on high cost support in
accordance with §54.3 18 of the FCC rules, FairPoint confirms that all state rates were above the rate floor
of $10.
(3)In accordance with 47 C.F.R. §54.304(d), Attachment 2 provides the details for the anticipated
CAF ICC receipts for FairPoint operations within the state. This attachment is being sent via overnight
delivery to the Commission under confidential seal.
(4)Also attached are copies of the certifications that were submitted to the FCC with FairPoint's
annual filing and documents in support of Transitional Intrastate Access Reductions and CAF ICC
Calculations. (Attachment 3)
