With that Order, the FCC began a transition to a national framework for certification of Eligible Telecommunications Carriers ("ETCs") and set forth a standard set of mfonnation that all ETCs must file by April 1 of each year. The Order also required ETCs to provide the same information to the respective state commissions. In subsequent orders, issued on February 3, and May 14, 2012, the FCC modified certain of the reporting requirements to comply with the federal Paperwork Reduction Act and revised its rules to moved the annual filing date to July 1 of each year For 2012, ETCs must file information that fulfills 47 C.F.R. 54.3 13(a)(2) through (a)(6) to the extent the information has been collected pursuant to state ETC certification requirements and 47 C.F.R. 54.3 13(h) by July 2, 2012. Qwest Corporation d/b/a/ CenturyLink hereby provides the information that fulfills 47 C.F.R. 54.313(h) to the Idaho Public Utilities Commisison as required by the USF/ICC Transformation Order. In addition, CenturyLink is providing the information that fulfills 47 C.F.R. 54.313(8). On April 2, 2012, CenturyLink filed the information responsive to 47 C.F.R. 54.3 13(a)(2) through (a)(6) with the Commission and will not be updating that information as a result of the July 2, 2012 FCC filing. Carriers shall now report lines and rates in effect as of June 1, 2012.
