Dear Ms. Dortch: Pursuant to Sections 63.71 and 63.19 of the Commission s rules, 47 C.R. ~9 63.71 and 63.19, Verizon Avenue Corp. (f/k/a OnePoint Communications Corp.), VIC-RMTS-, L.LC. OnePoint Communications-Georgia, LLC., and OnePoint Communications-Colorado, LL.C. (collectively, "Verizon Avenue ), all located at 12901 Worldgate Drive, Herndon, Virginia 20170, hereby provide notice that, effective May 1 , 2006, and subject to federal and state regulatory approval, Verizon Avenue will no longer provide local, intraLATA, interLAT A, and international telephone services in the following 28 states: Arizona, California, Colorado Connecticut, District of Columbia, Delaware, Florida, Georgia, Hawaii, Idaho, Illinois, Indiana Maryland, Massachusetts, Michigan, New Jersey, New York, North Carolina, Ohio, Oregon Pennsylvania, Rhode Island, South Carolina, Tennessee, Texas, Virginia, Washington, and West Virginia. Verizon Avenue currently has the following numbers of customers in these 28 jurisdictions: Arizona (80), California (454), Colorado (61), the District of Columbia (3), Florida (3), Georgia (20), Dlinois (693), Maryland (164), North Carolina (15), Ohio (278), Pennsylvania (15), South Carolina (12), and Virginia (113). Verizon Avenue has sent notices of the discontinuance to affected customers according to the requirements of Sections 63.71 (a) and 63.19 of the Commission s rules. See 47 C.R. ~~ 63.71(a)(1)-(5), 63.19(a)(1). Attached are copies of the sample customer notice letters that were sent via u.S. mail between February 27 and March 1, 2006. Verizon Avenue is non-dominant with regard to the services being discontinued. Dear Customer: We regret to infonn you that- subject to the approval of the (STATE Public Utilities Commission) Verizon Avenue Corp. ("Verizon Avenue ) will no longer be providing your local telephone service (and, if applicable regional toll and/or long distance telephone services) effective May 1,2006. Verizon Avenue s business plans have changed and we are in the process of withdrawing our competitive local exchange service certificate in (STATE). Your action is required! You must select a new local telephone provider no later than April 17, 2006 to avoid loss of your local telephone service. Please be aware that you are responsible for paying all bills rendered to you by Verizon Avenue during this transition. You may be subject to suspension or termination of your phone service in accordance with (S T ATE PDq rules if you fail to pay your telephone bill. Verizon Avenue s withdrawal from the local telecommunications market may also result in your loss of either (1) the ability to make directly dialed Regional Toll calls and/or Long Distance Toll calls or (2) the calling plan you have with your selected Regional Toll and/or Long Distance Toll provider. After selecting your new local telephone provider, we suggest that you also contact your current regional to11 and/or long distance service provider to ensure that your current regional toll and/or long distance calling plan is not changed as a result of your change in your local service provider. rfyou do not contact your regional toll and/or long distance provider, you may be charged basic rates (non-calling plan rates) for regional toll and/or long distance calls. IfVerizon Avenue is your regional toll and/or long distance provider, then you wlll need to select a new carrier. Subject to the approval of the (STATE Public Utilities Commission,) Verizon Avenue wlll not be providing regional toll or long distance services after May 1, 2006. If a deposit was required at the initiation of your service, deposits will be refunded at the time of disconnection. The deposit amount will be applied to the account bahince (if any) and any remaining credit will be refunded via check. Generally, you can find a list of most local telephone service providers in your local telephone directory. If you require assistance, please contact Verizon Avenue at 1-866-892-8368.
