Dear Ms. Jewell
Verizon Avenue Corp., an affiliate ofVerizon Communications Corporation, respectfully
requests that the certificate of authority granted in case no. GNR- T-03-, Order 29497
Certificate No. 428 be withdrawn. Verizon Avenue Corp. is no longer providing
intrastate telecommunications service in Idaho. Please note: Verizon A venue Corp. does
not have any customers in the state of Idaho.
Verizon Avenue Price Lists will be withdrawn concurrent with cancellation of the
certificate of authority
