Dear Mr.Cusick:
This letter is to request that the Idaho Public Utilities Commission notify the Universal ServiceAdministrativeCompany(USAC)and the Federal Communications Commission (FCC)thatPotlatchTelephoneCompanyiseligibletoreceivefederalhigh-cost support in accordance with theabove-referencedstatute and federal rule.
The amount of federal high-cost support Potlatch Telephone Company will receive in 2003 willcontinuetobeusedfortheservicesandfunctionalitiesoutlinedin47C.F.R.§54.101(a)and,as theattachedaffidavitshowsPotlatchTelephoneCompanycertifiesthatitwillonlyusethefederalhigh-cost support it receives for the provision,maintenanceand upgrading of facilities and serviceforwhichsuchsupportisintended.
This state certification for federal support will be an annual process.In order to receive federalsupportbeginningJanuary1ofeachyear,the Idaho Public Utilities Commission must file itsannualcertificationonorbeforeOctober1oftheyearbefore.
Potlatch TelephoneCompany respectfully requests that the Commission notify the FCC and USACpriortoOctober1ofthisyearthatPotlatchTelephoneCompanyiseligibletoreceivefederalhigh-cost support for 2003.

Affidavit of Business or Corporate Officer
Section 254(e)of the Telecommunications Act of 1996 requires that an eligible
telecommunications carrier receiving federal universal service support "shall use that support only for the
provision,maintenance,and upgrading of facilities and services for which the support is intended."In FCC
Order No.01-157 issued May 23,2001,the Federal Communications Commission concluded "that states
should be required to file annual certification with the Commission to ensure that carriers use universal service
support 'only for the provision,maintenance and upgrading of facilities and services for which the support is
intended'consistent with Section 254(e)."Fourteenth Report and Order,Twenty-Second Order on
Reconsideration,and Further Notice of Proposed Rulemaking in CC Docket No.96-45,and Report and Order
in CC Docket No.00-256,¶l87.Accordingly,the undersigned states and verifies under oath as follows:
1.I am an officer of TDS Telecom,parent Company of Potlatch Telephone Company
which is an eligible telecommunications carrier for receiving federal universal service support under
Section 214(e)of the Telecommunications Act of 1996.
2.All federal universal service support funds received by Potlatch Telephone Company during the 2003
calendar year will be used in a manner consistent with Section 254(e);that is,for the provision,
maintenance,and upgrading of facilities and services for which the support is intended.
3.This verification and affidavit is provided to the Idaho Public Utilities Commission to enable the IPUC
to certify to the Federal Communications Commission that federal universal support received by the
eligible carriers in the state will be used in a manner consistent with Section 254(e)of the
Telecommunications Act.
