In October 1999,the Commission issued Order No.28185 granting dPi-Teleconnect,LLC
a Certificate of Public Convenience and Necessity to resell prepaid local exchange services in Idaho.
In April 2000,Certificate No.372 was issued to dPi-Teleconnect.
On November 5,2012,dPi Teleconnect,LLC notified the Commission that it had changed
its name to Unity Telecom,LLC.
IT IS HEREBY CERTIFIED that this First Amended Certificate of Public Convenience
and Necessity,Certificate No.372,is issued to Unity Telecom,LLC to provide resold local exchange
services within the State of Idaho.The Commission approved this name change to Certificate No.372
by Order No.32683,in Case No.GNR-T-99-07.
