IT IS HEREBY CERTIFIED that this Certificate of Public Convenience and Necessity, Certificate No. 350, is issued to FirsTel, Inc. This Certificate authorizes FirsTel to provide Title 61 regulated telecommunications services, including basic exchange services, within certain parts of the state of Idaho. The Commission approved FirsTels Certificate by Order No. 27752 issued September 28, 1998, and authorized FirsTel to provide Title 61 services in the respective service areas of U S WEST Communications, as depicted in the map attached to this Certificate as Exhibit A. This Certificate is predicated upon and is issued pursuant to the findings of the Commission in Order No. 27752.
