A. Sheet Numbering Sheet numbers appear in the upper right
corner of the page. Sheets are numbered sequentially. When a new
sheet is added between sheets already in effect, a decimal isadded. For example, a new sheet added between sheets 14 and
would be 14.
B. Sheet Revision Numbers - Revision numbers also appear in the
upper right corner of each page. These numbers are used to
determine the most current sheet version on file with the IPUC.
For example, the 4th revised Sheet 14 cancels the 3rd revised Sheet14. Consul t the Check Sheet for the sheet currently in effect.
C. Paragraph Numbering Sequence There are nine levels of
paragraph coding. Each level of coding is subservient to its nexthigher level:

SECTION 1 - TECHNICAL TERMS AND ABBREVIATIONS
Access Line An arrangement which connects the Customer'
telephone to a Future Telephone Communications, Inc., designated
swi tching center or point of presence.
Authorized User - A person, firm, corporation , or any other entity
authorized by the Customer to communicate utilizing the Carrier'service.
Customer or End User - The person, firm, corporation or other
entity which orders, cancels, amends or uses service and is
responsible for payment of charges and compliance with the
Company's price list.
Company or Carrier - Future Telephone Communications , Inc., unless
otherwise clearly indicated by the context.
Equal Access - Where the local exchange company central office
provides interconnection to interexchange carriers with Feature
Group D circuits. In such end offices, customers presubscribe
their telephone line (s) to their preferred interLATA carrier.

FTC - Used throughout this price list to mean Future Telephone
Communications, Inc.
IPUC - Idaho Public Utilities Commission.
LEC - Local Exchange Company.
Special Access Origination/Termination - Where access between the
customer and the interexchange carrier is provided on dedicatedcircuits. The cost of these dedicated circuits is billed by the
access provider directly to the end user.
Switched Access Origination/Termination - Where access between the
customer and the interexchange carrier is provided on local
exchange company Feature Group circuits and the connection to the
customer is a LEC-provided business or residential access line.
The cost of switched Feature Group access is billed to the
interexchange carrier.
V & H Coordinates - Geographic points which define the originating
and terminating points of a call in mathematical terms so that the
airline mileage of the call may be determined. Call mileage is
used for the purposed of rating calls.

SECTION 2 - RULES AND REGULATIONS
Undertaking of Future Telephone Communications, Inc.
FTC's services and facilities are furnished for communications
originating at specified points wi thin the state of Idaho
under terms of this price list.
FTC installs, operates, and maintains the communications
services provided hereinunder in accordance with the terms and
condi tions set forth under this price list. FTC may act as
the Customer s agent for ordering access connection facilities
provided by other carriers or entities, when authorized by the
Customer, to allow connection of a Customer's location to the
FTC network. The Customer shall be responsible for all
charges due for such service arrangement.
The Company'services and facilities are provided on a
monthly basis unless otherwise provided , and are available
twenty-four hours per day, seven days per week.

Use
All facilities provided under this price list aredirectly or indirectly controlled by Future
Telephone Communications, Inc. and the Customer
may not transfer or assign the use of service orfacilities without the express written consent ofthe Company. Such transfer or assignment shall
only apply where there is no interruption of the
use or location of the service or facilities.
Prior written permission from the Company is
required before any assignment or transfer. All
regulations and conditions contained in this price
list shall apply to all such permitted assignees or
transferees, as well as all conditions of service.
Services provided under this price list may be used for any
lawful purpose for which the service is technically suited.

mistakes , interruptions , omissions , delays, errors,or defects in transmission which occur in the
course of furnishing service or facilities, in no
event shall exceed an amount equivalent to the
proportionate charge to the Customer for the period
during which the faults in transmission occur.
The Company shall not be liable for claim or loss,

consequential damage), for any interruption, delay,
error, omission, or defect in any service, facility
or transmission provided under this price list, if
caused by any person or entity other than the
Company, by any malfunction of any service or
facili ty provided by any other carrier, by an actof God, fire , war, civil disturbance , or act of
government or by any other cause beyond the
Company s direct control.

The Company shall not be liable for , and shall befully indemnified and held harmless by Customeragainst any claim or loss, expense , or damage

misappropriation or violation of any contract

the material, data, information, or other content
revealed to, transmitted, or used by the Company
under this price list; or for any act or omissionof the Customer; or for any personal injury or
death of any person caused directly or indirectlyby the installation, maintenance, location,condition, operation, failure, presence, use or
removal of equipment or wiring provided by the
Company, if not directly caused by negligence of
the Company.
No agent or employee of any other carrier shall be
deemed to be an agent or employee of the Company.
The Company shall not be liable for any defacementof or damages to the premises of a Customer
resul ting from the furnishing of service which is
not the direct result of the Company's negligence.
The above tariff language does not constitute a
determination by the Commission that a limit ofliabili ty imposed by the Company should be upheld
in a court of law. Acceptance for filing by the
Commission recognizes that it is a court'
responsibility to adjudicate negligence and
consequential damage claims. It is also the
court's responsibility to determine the validity of
the exculpatory clause.

Deposits
The Company does not collect deposits from Customers.
Advance Payments
For Customers whom the Company determines an advance payment
is necessary, FTC reserves the right to collect an amount not
to exceed one ( 1 ) month's estimated charges as an advance
payment for service. This will be applied against the next
month's charges and a new advance payment may be collected for
the next month.
Taxes
All state and local taxes (i. e., gross receipts tax, sales
tax , municipal utilities tax) are listed as separate line
items and are not included in the quoted rates.
Terminal Equipment
The Company s facilities and service may be used with or
terminated in Customer-provided terminal equipment or
Customer-provided communications systems, such as a telephone
set, PBX or key system. Such terminal equipment shall be
furnished and maintained at the expense of the Customer,
except as otherwise provided. The Customer is responsible for
all costs at his or her premises , including personnel, wiring,
electrical power , and the like , incurred in the use of the
Company's service. When such terminal equipment is used, the
equipment shall comply with the generally accepted minimum
protective criteria standards of the telecommunications
industry as endorsed by the Federal Communications Commission.

Installation and Termination
Service is installed upon mutual agreement between the
Customer and the Company. The service agreement does not
al ter rates specified in this price list.
2. 10 Payment for Service
The Customer is responsible for payment of all charges forservices and equipment furnished to the Customer or to an
Authorized User of the Customer by FTC. All charges due by
the Customer are payable to the Company or to the Company'
authorized billing agent. Terms of payment shall be according
to the rules and regulations of the billing agent and subject
to the rules of regulatory agencies, such as the IPUC. Any
objections to billed charges must be reported to the Companyor its billing agent. Adjustments to Customer's bills shallbe made to the extent that circumstances exist which
reasonably indicate that such changes are appropriate.
11 Cancellation by Customer
Customer may cancel service by providing 30 days written
notice to the Company.
12 Interconnection
Service furnished by FTC may be connected with the services orfacili ties of other carriers. Such service or facilities, if
used, are provided under the terms, rates and conditions ofthe other carrier. The Customer is responsible for all
charges billed by other carriers for use in connection with
FTC's service. Any special interface equipment or facilities
necessary to achieve compatibility between carriers is the
responsibili ty of the Customer.

13 Refusal or Discontinuance by Company
FTC may refuse or discontinue service under the following
conditions. unless otherwise specified, the Customer will be
given five (5) d~ys written notice and allowed a reasonable
time to comply with any rule or remedy any deficiency.

municipal law , ordinance or regulation pertaining to
telephone service.
For the use of telephone service for any other property
or purpose other than that described in the application.

14 Inspection, Testing and Adjustment

maintenance. No interruption allowance will be granted for
the time during which such tests and adjustments.

15 Tests, Pilots, Promotional Campaigns and Contests
The Carrier may conduct special tests , pilot programs, waivers
and promotions to demonstrate the ease of use , quality of
service and to promote the sale of its services.
2. 16 Interruption of Service
Credi t allowances for interruptions of service which are not
due to the Carrier's testing or adjusting, to the negligence
of the Customer, or to the failure of channels, equipment or
communications systems provided by the Customer, are subject
to the general liability provisions set forth in Section 2.herein. It shall be the obligation of the Customer to notify
Carrier immediately of any interruption in service for whicha credit allowance is desired by Customer. Before giving such
notice, Customer shall ascertain that the trouble is not
wi thin his or her control , or is not in wiring or equipmentif any, furnished by Customer and connected to Carrier'terminal. Interruptions caused by Customer-provided or
Carrier-provided automatic dialing equipment or access-code
programmed PBXs are not deemed an interruption of service as
defined herein since the Customer has the option of using the
long distance network via local exchange company access.

SECTION 3 - DESCRIPTION OF SERVICE
General
Future Telephone Communications, Inc. offers long distance
telecommunications services to residential and business
Customers. Intrastate service is provided only to Customers
of FTC's companion interstate offerings. Descriptions and
rates contained in this tariff apply to the intrastate portion
of the Carrier's services.
Services provided by FTC include direct dialed (1+) service
and travel card service. Direct dial services are availablevia Switched Access Origination or Termination. When a
Customer elects to use Travel Card service, calls may be
initiated from any location from which the caller can dial the
appropriate access codes.

Calculation of Distance
Usage charges for all mileage sensitive products are based on
the airline distance between the serving wire center locations
associated with the originating and terminating points of thecall.
The distance between the originating and terminating points is
calculated by using the "V" and "H" coordinates of the servingwire centers as defined by BellCore (Bell Communications
Research), in the following manner:
Step 1 - Obtain the "V" and "H" coordinates for the servingwire center of the Customer switch and the destination
point.
Step 2 - Obtain the difference between the "V" coordinates of
each of the Rate Centers. Obtain the Difference between the
"H" coordinates.
Step 3 -Square the differences obtained in Step
Step 4 - Add the squares of
difference obtained in Step

Step 5 - Divide the sum of the square obtained in Step 4 byten (10). Round to the next higher whole number if any
fraction results from the division.
Step 6 - Obtain the square root of the whole number obtained
in Step 5. Round to the next higher whole number if any
fraction is obtained. This is the distance between the
originating and terminating serving wire centers of the call.

Timing of Calls
Billing for calls placed over the network is based in part on
the duration of the call.

Long distance usage charges are based on usage of
FTC's service. Usage timing begins upon receipt of
answer supervision from the called party at thedistant end. Call duration timing will commence
from that point.
Chargeable time for a call ends upon disconnection
by either party.
In calculating the total time for a particular
call , any fraction of the last increment of use, is
rounded up to the next higher increment.
Minimum call durations, usage measurements and
rounding increments are specified on a per-product
basis in Section 4 of this tariff.
No charges apply for incomplete calls. If a
Customer believes he or she has been incorrectly
billed for an incomplete call, the Company will,
upon notification, investigate the circumstances of
the call and issue a credit when appropriate.

period for $0.01 per minute. Usage exceeding thirty (30)
minutes will be billed at the rates in effect as described inthis tariff.

FTC offers " 1+" outbound and travel card services to its
Customers. A variety of rate plans, as listed in Section 4 ofthis tariff, are available. Rates may vary by service type,
time of day, day of week, distance, and calling volume.
Service offerings available from the carrier are:
FTC Option One
FTC Option One is a Message Telecommunications
Service available to residential and small business
Customers. Calls are billed on the Customer's Local
Telephone bill through third party billing
arrangements. Calls are billed in six second
increments. There is no billing for calls by
residential customers under 30 seconds.

FTC option Two is a discounted flat rated service
available to residential and small business
customers who commit to over $10.00 per month.
Calls are billed on the Customer's Local Telephonebill through third party billing arrangements.

Each Customer is charged individually for each call placed
through the Company. Charges may vary by service offering,
class of call, time of day, day of week and/or call duration.
Customers are billed based on their use of FTC's long distance
service. No installation charges or fixed monthly recurringcharges apply unless otherwise specified.
