On October 5, 2016, Fremont Telcom Co. notified the Commission that the Company
has changed its brand name to Fremont Telcom Co. dba Blackfoot.
IT IS HEREBY CERTIFIED that this First Amended Certificate of Public
Convenience and Necessity, Certificate No. 333, is issued to Fremont Telcom Co. dba Blackfoot.
This Certificate authorizes Blackfoot to provide Title 61 regulated telecommunications services,
including basic exchange services, within parts of the State of Idaho. The Commission
approved this name change to Certificate No. 333 by Order No. 33624 issued October 13,
2016, in Case No. FRE-T-97-01. Blackfoot is authorized to provide Title 61 services in the
