TO AMEND ITS ) CASE NO. FRE-T-97-01 CERTIFICATE OF PUBLIC CONVENIENCE ) AND NECESSITY NO. 333 TO REFLECT ITS ) NAME CHANGE TO FREMONT TELCOM ) ORDER NO. 33624 CO. DBA BLACKFOOT ) -------------------) On October 5, 2016, Fremont Telcom Co. notified the Commission that the Company has changed its brand name to Fremont Telcom Co. dba Blackfoot. The Company provides resold and facilities-based basic local exchange service within parts of Idaho under Certificate of Public Convenience and Necessity No. 333. The Company has asked the Commission to update the CPCN to reflect the Company's new name. With this Order, we grant the Company's request. BACKGROUND In late 1997, the Commission authorized Fremont Telcom Co. to provide resold and facilities-based basic local exchange services within parts of Idaho under Certificate No. 333. See Order No. 27253. The Company has now notified the Commission that it has changed its brand name to Fremont Telcom Co. dba Blackfoot. The Company also filed a copy of the Certificate of Assumed Business Name with the Idaho Secretary of State's office. Staff has reviewed this documentation and recommended that the Commission amend Certificate No. 333 to reflect the name change to Fremont Telcom Co. dba Blackfoot. FINDINGS Based on our review of the Company's filing and Staff's recommendation, the Commission finds it appropriate to amend Certificate No. 333 to reflect that the Company has changed its brand name to Fremont Telcom Co. dba Blackfoot. ORDER IT IS HEREBY ORDERED that Certificate No. 333 is amended to reflect that the Certificate holder's name is Fremont Telcom Co. dba Blackfoot. ORDER NO. 33624 1 THIS IS A FINAL ORDER. Any person interested in this Order may petition for reconsideration within twenty-one (21) days of the service date of this Order with regard to any matter decided in this Order in Case No. FRE-T-97-01. Within seven (7) days after any person has petitioned for reconsideration, any other person may cross-petition for reconsideration. See Idaho Code§§ 61-626 and 62-619.
