On October 5, 2016, Blackfoot Communications, Inc. dba Fremont Communications
notified the Commission that the Company has changed its brand name to Blackfoot
Communications, Inc. dba Blackfoot.
IT IS HEREBY CERTIFIED that this Second Amended Certificate of Public
Convenience and Necessity No. 370 is issued to Blackfoot Communications, Inc. dba Blackfoot.
This Certificate authorizes Blackfoot to provide local exchange and interexchange services
within the State of Idaho. The Commission approved this name change to Certificate No. 370 by
Order No. 33625 issued October 13, 2016, in Case No. FRC-T-99-01.
