IT IS HEREBY CERTIFIED that this Amended Certificate of Public Convenience
and Necessity No.370 is issued to Blackfoot Communications,Inc.dba Fremont
Communications.This Certificate authorizes Fremont Communications to provide local
exchange and interexchange services within the State of Idaho.The Commission approved this
name change to Certificate No.370 by Order No.33127 issued September 16,2014,in Case No.
FRCT-99-0 1.
