BACKGROUND AND DISCUSSION
On October 5, 2016, Blackfoot Communications, Inc. filed its Application to change its
name to reflect its brand name change. The Company's registered name is now Blackfoot
Communications, Inc. d.b.a. Blackfoot. This revision will be the second amendment to
Certificate No. 3 70 that was issued in Case No. FRC-T-99-01.
