NAME ON ITS CERTIFICATE OF PUBLIC CONVENIENCE AND NECCESITY
Blackfoot Communications, Inc. ("Blackfoot") hereby submits this request to the
Idaho Public Utilities Commission ("Commission") seeking to change the d/b/a name
associated with its certificate of public convenience and necessity to provide facilities­
based competitive local exchange and resold interexchange telecommunications
services in the state of Idaho as allowed for under state law and pursuant to
Commission rule as set out in Idaho Code§ 62.
Attached as "Exhibit A" is the announcement to be provided to customers of the
name change that will be provided as a billing insert along with a billing statement
message. "Exhibit B" is a copy of the Assumed Business Name Filing made with the
Idaho Secretary of State.
Blackfoot Communications, Inc. formerly d/b/a Fremont Communications is a
facilities-based CLEC providing voice and data service in Southeastern Idaho and West
Yellowstone, Montana. Blackfoot Communications, Inc. formerly d/b/a Blackfoot
Communications, Inc. is a facilities-based CLEC providing voice and data services
throughout Montana. As a result of its branding change Blackfoot Communications, Inc.
will be doing business as "Blackfoot" in all of its markets in the same fashion as it
operates today.
