broadband infrastn"rcttue tax oedit authorized by Section 63-3029l,Idatro Code. In support of its
Application, FMTI states as follows:
1. FMTI is a provider of ba.sic local exchange service, broadband
telecommunications service and other telecommunications services in Idaho.
2. During the calendar year 202L, FMTI made certain inveshnents that constitute
"qualified broadband equipment" within the meaning of Section 63-3029(IX3)(b), Idaho Code.
Exhibit attached hereto, describes FMTI's qualified broadband equipment and contains the
information and representations required by this Commission's Order No. 35297 in Case No
GNR-T-21-10.
