(.Commission) order certiffing that certain telecommunications equipment is eligible for the
broadband inftastructure tax credit authorized by Section 63-3029l,Idaho Code. In zupport of its
Application, FMTI states as follows:
l. FMTI is a provider of basic local exchange senrice, broadband
telecommunications senrice and other telecommunications services in Idaho.
Z. During the calendar yew 2020, FMTI made certain investnents that constitute
*quatified broadband equipmarf'within the meaning of Section 63-30290(3)O), Idatro Code.
Exhibit A, attached hereto, describes FMTI's qualified broadband equipment and contains the
infonnation and representations required by this Commission's Order No. 28784 in Case No.
GNR-T-O1-10.
