Commission (.Commission') order cemiffing that certain telecommunications equipme'nt is
eligible for the broadband infrastructure tax credit authorizd by Section 63-3029l,Idalro Code'
ln support of its Application, FMTI states as follows:
l. FMTI is a provider of basic local exchange service, broadband
telecommunications service and other telecommunications services in Idaho.
2. tn the original filing by FMTI in this matter, the list of qualified broadband
equipment was not correct, and included the qualified broadband oquipment of an affiliated
entity rather than that of FMTI. In this Amended Application, FMTI conects that error'
3. Attached hereto as Exhibit A, is the conected list of qualified broadband
equipment within the meaning of Section 63-3029(IX3)O), Idaho Code, in which FMTI invested
during the cale,lrdar year 201g, FMTI. Exhibit A, contains the information and represe,ntations
requked by this Commission's Order No. 28784 in Case No. GNR-T-01-10'
