DBA XMT3 TO AMEND ITS CERTIFICATE OF PUBLIC CONVENIENCE AND NECESSITY NO. 455 TO REFLECT ITS NAME CHANGE TO CLEAR VOICE TELECOM, INc. CASE NO. EXT-05- ORDER NO. 30923 On March 7 2006, the Commission issued Order No. 29990 granting Extreme Media Technologies, Inc. dba XMT3 a Certificate of Public Convenience and Necessity to provide local exchange and interexchange services within the State of Idaho. On March 10, 2006, Certificate No. 455 was issued to Extreme Media Technologies, Inc. dba XMT3. In October 2008, Extreme Media notified the Commission that it had changed its name from Extreme Media Technologies, Inc. (XMT3) to Clear Voice Telecom, Inc. The name on its Certificate was not changed at that time, however, because the Company had not changed its name on file with the Secretary of State, nor did it file a revised price list with the new name. Apparently, the Company has gone through a number of management and organizational changes over the past year, but has recently resolved the outstanding issues needed to bring it into compliance with filing requirements. On September 17 2009, the Company filed a request for name change to Clear Voice Telecom, Inc. The Company also filed its Amended Certificate of Authority from the Idaho Secretary of State and revised price list. Based on our review of the Application, the Commission approves Extreme Media request to amend its Certificate of Public Convenience and Necessity No. 455 to reflect the change from Extreme Media Technologies, Inc. dba XMT3 to Clear Voice Telecom, Inc. ORDER IT IS HEREBY ORDERED that Certificate of Public Convenience and Necessity No. 455 be amended to reflect the name change from Extreme Media Technologies, Inc. dba XMT3 to Clear Voice Telecom, Inc. THIS IS A FINAL ORDER. Any person interested in this Order may petition for reconsideration within twenty-one (21) days of the service date of this Order with regard to any ORDER NO. 30923 matter decided in this Order in Case No. EXT-05-01. Within seven (7) days after any person has petitioned for reconsideration, any other person may cross-petition for reconsideration. See Idaho Code ~~ 61-626 and 62-619.
