Comes now, Clear Voice Telecom, Inc., formerly known as Extreme Media
Technologies, Inc., and by and through undersigned counsel, hereby submits this Application
to change its name on its Certificate of Public Convenience and Necessity from "Extreme
Media Technologies, Inc." to "Clear Voice Telecom, Inc." This application is made pursuant
to Rule 202 of the Rules for Telephone Corporations Subject to the Regulation of the Idaho
Public Utilties Commission Under the Telecommunications Act of 1988.
BACKGROUND
Extreme Media received its Certificate of Public Convenience and Necessity on March
7,2006, pursuant to Order No. 29990. On November 13, 2007 Extreme Media officially
changed its name to Clear Voice Telecom, Inc. See Clear Voice Exhibit I, attched hereto.

Access Line
The circuit that travels from the CLEAR VOICE TELECOM, INC.'S Central Offce to
the Subscriber's premise termina,ting at the Network Interface Device (NID), which
provides direct access to the local exchange and the toll switching netorks.
Circuit
A Chael used for the transmission of electrcal energy in the fushing of
telephone service.
Company
Clear Voice Telecom, Inc.
Connecting Company.
A corporation, association, parership, LLC, or individual owng or
operating one or more exchanges and with who traffc is interchanged.
Contract
The service agreement between a subscriber and the Company under which services and
facilities are fushed in accordance with the provisions of the Price Lists applicable.
Customer
See "Subscriber", below.
Customer Provided Equipment (CPE)
Devices, apparatus and their associated wig provided by a Subscriber for use with
facilities fushed by the Company.
Direct Dialing
The capability for a Subscriber to dial anywhere in the United States with a series of
numbers without operator assistace.
Exchange Area
The terrtory served by an Exchange.

Extension Station
An additional station connected on the same circuit as the main station and having the
same telephone number as the main station.
Extr Listing
Any listig of a name or information in connection with a Subscriber's telephone number
beyond that to which he is entitled in connection with his reguar service.
InsUùlation Charge
A Ilon-recurng charge made for the placing or fuishing of telephone equipment, which
may. apply in pla.ce of or in .addition to Service Connection Chages and other a.pplica.ble
charges for service or equipIIent.
Local Chanel
That portion of a chanel which connects a station to the interexchange chanel; it also
applies to a chanel connecting two or more stations withn an exchange area.
Local E£íchige Servce
Telephone servce fuished between telephone station and the telephone station(s)
of others located with the same local servce area.
Local Message
A communication between telephone stations withn the same Exchange Area.
Local Service Area
That geogrphical area thoughout which a Subscriber obtans telephone service without
the payient of a toll charge.
Mai Station
A suital:le telephone intrent or station that is connected to a networka.cess line
though a Central Office and has a unique telephone number.
Premises
All of the buildig or the adjoining portions of a buildig occupied and used by the
Subscriber; or all of the buildings occupied and used by the Subscribe as a place of

Price List
The document filed by the Company with the Public Utilties commission which lists the
communcation services offered by the Company and the associated prices.
Primar Station
Synonymous with Main Station.
Private Brach Exchange (PBX)
An arangement of equipment used by a Subscriber and connected directly to a central
office by means of tr access lines, from which connection is made to stations at
various locations or Subscriber premises, thereby providing telecommuncations between
these stations and also communcation with the general exchange system.
Private Line
A circuit provided to fush communcation only between the two or more telephones
directly connected to it, and not having connection with either centr office Or P .BoX.

Station
(see Telephone Station)
Subscriber
A person or agency subscribing for Clear Voice Telecom, Inco's telephone service. As
used in this Price List, a separate. Subscriber is involvedat each location, or continuous
property, where service is fuished. One individua or finn therefore may be considered
as two or more separate Subscribers, even in the same Exchange. The privileges,
restrictions, and rates established for a Subscriber to any class of service are limited to the
service at one location; no group treatment of service at separate locations fushed to
one individua or firm, is contemplated or to be implied, except when definitely provided
for in the schedules.
Telephone Station
A suitable telephone instrent, consisting of a tranmitter, receiver, and associated
apparatu, so connected as to permit transmitting of and receiving telephone messages.

Tie Tru
A circuit comiecting two P.B.X. systems for the purpose of intercommuncating between
the stations connected with such P.B.X. switching appaatu. The circuit is not intende
to provide for general exchange service though either of the P .B.x. systems with which
it connects.
Toll Message
A messae from a calling station to astation located in a different local service ara.
Toll Service
Telephone servce rendered by the Company between patrons in different loca service
areas in accordace with the rates specified in the Company's Toll Pnce List.
Tru
A telephone communcation chanel between (a) two rans of switchig equipment in the
same central offce, (b) between central office unts in the sa;e switcb.gcenter, or (c)
between two switching centers.
GENERA RULES AND REGULATIONS
A. APPLICATION
The rues and regia.tions speci.ed herein apply to the intrastate services and facilitieS of
CLEAR VOICE TELECOM, INC. hereinafer referred to as "the Company". Failure on
the par of the Subscribers to observe these terms and conditions of the Company, after
due notice of such failure, automatically gives the Company the. authority todiscontiue
the fushig of service.
B. EXPLANANTION OF SYMBOLS
(C) Signfies a changed regulation.
(D) Signfies a discontinued rate, treatment or regiation.
(I) Signfies an increased rate or new treatment resulting in increased rate.
(N Signifies a new rate, treatment or reguation.

C. OBLIGA nON OF COMPANY
1. Availability of facilities
The Company's obligation to fuish telephone servce is depende.nt upon its
abilty to secure suitable facilities and to provide such service without uneasonable
expense.
2. Interrption of Servce
The Company will credit a Subscriber's account in accordance with Telephone Customer
Relations Rule 503. IDAPA 31.41.01.503.
3. Directory Errors and Omission
The Company endeavors to correctly list Subscribers, their telephone numbers and other
information in the local telephone directory.
4. Defacement of Premises
The Company will repair or replace any defacement or damge of property due to
instlation, existence, or removal of Company propert, when the damge is the result of
negligence of the Company. .
5. Adjustrent of Charges
In case of over-biling, a refud will be made by the Company for the full
amount of excess chages when the amount can be determed; when the amount caot
be determined from available records, the maximum refud will not exceed the
estimated over-biling over a thee-year period.
In case of under-biling, the Company reserves the right toback-bil for the
deficiency charges up to a period of thee years.

D. USE OF SERVICE AND FACILITIES
1. Ownersl)p. and Use of Equipinent
All equipment and lines fushed by the Company are the propert of the
Company unless otherwse speified. Company agents or employees shall have the right
to enter Subscriber premises at any mutuly agreed upon reasonable hour to instal,
maintan or remove, as appropriate, Company equipment.
The Coinpaty may refuse to intal or ma4tan any service at locatiol1 tPt are
hazardous to Company employees. If such service is fuished, the Subsçrnermay. be
required to instal and maintan such service, holding the Company harless from any
claims for damage by reason of the instalation and maitenance of ths servce.
2. Interconnection Policy
CPE may be used and Subscriber provided communication systems may be
connected with the facilties fushed by the Company for teleconuuncations servces
subjectto terms and .coliditiol1s outlined in other pa of ths Price List. Incae any
unauorized.attchment is made, the Company shall have the right to disconnect,
suspend, or tennináte the servce.
3. Use of Subscriber. Servce
Subscriber telephone servce is fushed only for the use by the Snbsril:r,his
family and associates. The Company may refuse to install or permt such servce to
remai on premises of public or semi-public character.
The equipmtmt may be instaled, at such locations provided the service is located
so it is not accessible for public use.
4. Tampering with Equipment
The Company may refuse to fuish telephone service when company equipment
shows any evidence of taperng for the purose of obtanig seice without payment of
chages applicable to the serce rendered. by the Company.

5. Use of Improper Language or Impersonation of Another
The Company may refuse service to anyone who uses or penn.its abusive or
obscene language over Company facilities or impersonates another individual with
fraudulent or malicious intent.
6. Indiscriinate Use of Facilties
The Company may refue to fush service or requie upgrading of services
provided to any Subscriber who allows indiscriminate use of Company facilties except in
case of emergency.
E. ESTABLISHMENT AND FURNISHING OF SERVICE
l. Application for Service
Application for servce mus be made on the Company's stad~d foni, which
becomes a contract when accepte in wrtig by the Company or upønestablishment of
service. The person or entity submittg an application for service is hereafer referred to
as "Applicant". The conditions of such contracts are subject to all provisions of ths and
other applicable Price Lists. Requests for additional service may be made orally, if
provided in the original contract, and no advance payment will be required. A move
within the exchange area is not considered to terminate the contract and orders for such
may be made orally.
2. Telephone Numbers
The Company may chage the telephone number or central offcedesigiion of a
Subscriber whenever it is requied WITH consent FROM TH SUBSCRIBER and 30
days advance wrtten notice.
3. Alterations
TlieSubscriber agees to notify the Company .of any alterations that will
necessitate changes in the Company's wig; and the Subscriber. agr~eS . to pay. .the
Company's curent charges for such chages. The Subscriber can change the inside
wiring up to the demarcation point.

4. Payment of Servìce
The Subscriber is requied to pay all chages for services rendered by the
Company, both exchange and toll, in accordace with provisions contained elsewhere in
this Priçe List.. T1le Sttbse.riber is respqnsible for all chages for services .:renø;ered at
hislher telephone, including collect charges.
5. Maintenance and Repairs
The Company shall bea the expense of all repair and maitenance of its facilties,
except where daage. or destction of its facilties is due to .the gross neglect of the
Subscriber. The Subscriber may not rearge, remove or disconnect any Compay
facilties without consent of the Company.
6. Line Extensions

H. MINIMUM CONTRACT PERIODS AND TERMINATION OF
SERVICE
1. Minium Contract Periods
Except as hereinafer provided, the mium contrac period for all services and
facilities are one month at the same location.
The COmpany may require a minimum contract period IQnger tban one month at
the samelQc¡:tion, in, cQtme.ction with speial (non-stadard) types or argemen,ts of
equipment, or for unusua constrction necessar to meet special demands, and involving
extra cost.
2. Termination Of Service
The Company shall comply with all applicable Commssion rues relating to the
denial, restriction and notification processes before denying, restrcting or termin,ating a
Subscriber's service. Specifically, the Company shall abide by Telephone Customer
Rel¡:tion,Riies 301 thoug 314. IDAPA 31.41.01.301- 314.
1. PAYMENT FOR SERVICE AND FACILITIES
The Subscriber shall pay for serce and facilities monthy in advance, except all
varous unts of governent, and shall pay for Moves and Changes wheribiled. Failure
to receive a bil does not relieve the Subscrber of the responsibility for payment in
accordance with the provisions set fort herein.
All bils for service are due and payable at the offce of the Compåty On or
before the fifteenth (15th) day following the post marked date of the statement of the
month in which the bil is rendered.
J. SPECIAL SERVICES AND FACILITIES
Special servces and facilties not ordiarly used in the fuishing of telephone
servce and not otherwise provided for by the Price List schedules of the Company may
be fushed or leased pursuat to special contrt for such special serice or facility for
such period as may be agreed upon, provided such speial serce or facility or the use
made thereof is not unawf and does not interfere with the telephone service fushed
by the Company. Special services are provided for each individua application as a
custom-engineered system to satisfy and provide for the needs of that Subscriber.
Applicable charges will be determined by the Company for each individua system. In

devices sequentially or simultaneously.
Thee-Way Callng
Pennits a telephone user to add a thrd par to an existing local or long distace ~i. It
will also pennit a telephone user to consult prvately with a thrd par while holding the
origina calL.
Speed Calling
Penits placing local and long distace calls to pre-selected telephone numbers by dialing
an abbrviated code. To add a number to the callng list the Subscriber uses his own
telephone.

OBLIGA nON OF COMPANY
The Company shall be held harless by the Subscriber in cases where the Subscriber's
telephone number is transmitted via the Caller ID CLASS Feature to another Subscriber who
subscribes to service, and the Subscriber has not blocked the transmission of his /her telephone
number.
2. Obligation ofthe Subscriber
Under no circumstances should any Subscriber to the Caller ID Custom Callng Feature
use telephone numbers delivered to him/her for purposes of marketing any service, or for the sale
of the numbers to any interested party. Permitted uses of the number information received
through the CID service include:
"billng and collection, routing, screening, and completion of the originating Subscriber's
core transaction, or for services directly related to the originating
Subscriber's call or transaction."
Caller ID information can b.e used only to market goods and services to existing
Subscribers, and only to market goods and services to existing Subscribers that are directly
related to those the existing Subscriber already uses.
Notification of ilegal procedures having been given herein, the Company shall be held
harmless by all parties in cases where Subscribers of the Company use this information in an
unauthorized manner, as described above.
