On July 15, 1998, the Commission issued Order No. 27627 granting Electric
Lightwave, Inc. (ELI) Certificate of Public Convenience and Necessity to provide local
exchange telecommunication services within the State of Idaho. Certificate No. 344 was issued
on July 17, 1998.
On April 16 2004, ELI filed a letter notifying the Commission that it had changed its
structure from a corporation to a limited liability company. ELI requested a name change from
Electric Lightwave, Inc. to Electric Lightwave, LLC. ELI also included with the letter an
updated price list reflecting the change and updated contact information. Commission Staff
recommended the Commission approve the request for name change.
Based on our review of the Application, we find it is reasonable to amend Certificate
of Public Convenience and Necessity No. 344 to reflect the change in name from Electric
Lightwave, Inc. to Electric Lightwave, LLC.
ORDER
IT IS HEREBY ORDERED that the name on Certificate of Public Convenience and
Necessity No. 344 be amended to reflect the change in name from Electric Lightwave, Inc. to
Electric Lightwave, LLC.
THIS IS A FINAL ORDER. Any person interested in this Order may petition for
reconsideration within twenty-one (21) days of the service date of this Order. Within seven (7)
days after any person has petitioned for reconsideration, any other person may cross-petition for
reconsideration. See Idaho Code ~ 61-626.
