PRICE LIST FORMAT
Page Numbering - Page numbers appear in the upper right comer of the page. Pages are numbered
sequentially. However, new pages are occasionally added to the price list. When a new page is added between
pages already in effect, a decimal is added. For example, a new page added between pages 14 and 15 would
be 14.
Page Revision Numbers - Revision numbers also appear in the upper right comer of each page. These
numbers are used to determine the most current page version on file with the Commission. For example, the
4th revised Page 14 cancels the 3rd revised Page 14. Because of various suspension periods, deferrals, etc.
the Commission follows in their price list approval process, the most current page number on file with the
Commission is not always the price list page in effect. Consult the Check Sheet for the page currently in
effect.
Paragraph Numbering Sequence - There are nine levels of paragraph coding. Each level of coding is
subservient to its next higher level:

Check Sheets - When a price list filing is made with the Commission, an updated check sheet accompanies
the price list filing. The check sheet lists the pages contained in the price list, with a cross reference to the
current revision number. When new pages are added, the check sheet is changed to reflect the revision. All
revisions made in a given filing are designated by an asterisk(*). There will be no other symbols used on this
page if these are the only changes made to it (i., the format, etc. remains the same, just revised revision
levels on some pages). The price list user should refer to the latest check sheet to find out if a particular page
is the most current.

SECTION 1 - TECHNICAL TERMS AND ABBREVIATIONS
Access Line - An arrangement which connects the Customer’s location to the Carrier's designated point
of presence or network switching center.
Aggregator - Any person, excluding local exchange carriers and cellular service providers, that, in the
ordinary course of its operations, make telephones available to the public or to transient users of its premises,
for intrastate telephone calls using a provider of operator services.
Authorization Code - A pre-defined series of numbers to be dialed by the Customer or End User upon
access to the Company's system to notify the caller and validate the caller's authorization to use the services
provided. The Customer is responsible for charges incurred through the use of his or her assigned
Authorization Code.
Authorized User - A person, firm or corporation, or any other entity authorized by the Customer or
Subscriber to communicate utilizing the Company's services.
Available Usage Balance - The amount of usage remaining on a Debit Account at any particular point in
time. Each Account or Card has an Initial Account Balance or credit to be debited which is stated either in
U.S. dollars or Call Units, depending upon the type of service. The Available Balance or credit is depleted
as services provided by the Company are utilized by the Customer.
Billed Party - The person or entity responsible for payment of the Company's Service(s): For a Direct
Dialed Call, the person or entity responsible for payment is the Customer responsible for payment for local
telephone service at the telephone used to originate an intrastate call. In the case of a Traveler Card call,
Phone Home Card call or other credit card call (herein collectively the ("Card"), the person or entity
responsible for payment is the Customer of record of the Traveler Card, Phone Home Card or other valid
and acceptable Card used. In the case of a collect or third party call, the person or entity responsible for
payment is the person responsible for payment for local telephone service at the telephone number that agrees
to accept charges for the call. In the case of a Room Charge Call, the entity responsible for payment is the
Aggregator controlling the telephone used to originate the intrastate call. In all Operator Assisted calls not
involving Cards, third party calls, collect calls or Room Charge calls, the person or entity responsible for
payment is the Customer responsible for payment for local telephone services at the telephone used to
originate the intrastate call.
Calling Card Call - A direct dialed or Operator Assisted call for which charges are billed not to the
originating telephone number, but to a LEC or interexchange carrier calling card.

Commission - Refers to the Idaho Public Utilities Commission.
Company - Refers to Dancris Telecom, L.L.C. ("Dancris"), issuer of this tariff.
Credit Card Call - A Direct Dialed or Operator Assisted call for which charges are billed not to the
originating telephone number, but to a credit commercial card, such as Visa or MasterCard.
Customer - The person, firm or corporation, or other entity which orders, cancels, amends, or uses service
and is responsible for the payment of charges and/or compliance with tariff regulations.
Customer Premises Equipment - Terminal equipment, as defined herein, which is located on the
Customer's premises.
Dancris - Used throughout this tariff to refer to Dancris Telecom, L.L.C. unless otherwise clearly indicated
by the context.
Debit Account - An account which consists of a prepaid usage balance depleted on a real-time basis during
each Debit Service call.
Debit Account Payment - A payment by commercial credit card, check, or draft that increases or
establishes the Available Usage Balance.
Debit Card - A card issued by the Company which provides the Customer with a Personal Identification
Number or Authorization Code and instructions for accessing the Carrier's network.
Debit Service Call - A service accessed via a "1-800" or other access code dialing sequence whereby the
Customer or Authorized User dials all of the digits necessary to route a call. Network usage for each call is
deducted from the available usage balance on a Company issued Debit Account.
Depletion - Real time reductions in the Available Usage Balance, based on usage of the customer Debit
Account.
Direct Dialed Call - An intrastate telephone call that is automatically completed and billed to the telephone
number from which the call originated without the automatic or live assistance of an operator.

Initial Usage Balance - The amount of usage on a Debit Account upon issuance or activation and before
any depleting call activity.
LEC - Local Exchange Company.
Marks - A collective term to mean such items as trademarks, service marks, trade names and logos;
copyrighted words, artwork, designs, pictures or images; or any other device or merchandise to which legal
rights or ownership are held or reserved by an entity.
Operator Assisted Call - An intrastate telephone connection completed through the use of the Company's
operator.
Operator Service Charge - A non-measured (fixed) charge that is added to a measured charge in
calculating the total tariff charges due for a completed Operator Assisted Call.
Operator Services - Any telecommunication service that includes, as a component, any automatic or live
assistance to a Customer or its Authorized User to arrange for billing or completion, or both, of an intrastate
interLATA telephone call through a method other than:
(i)automatic completion with billing to the telephone form which the call originated; or
(ii)completion through an access code used by an Authorized User, with billing to an account
previously established with the carrier by the Authorized User.
Operator Service Provider ("OSP") - Any person or entity that provides operator services by using either
live or automated operator functions. When more than one entity is involved in processing an operator service
cal, the party billing the calls shall be considered the OSP. However, subscribers to customer-owned pay
telephone service shall not be deemed to be an OSP.
Operator Station Calls - An Operator Assisted call wherein the person originating the call is assisted by
an operator but does not specify a particular person, department or extension to be reached through a PBX
attendant.

Personal Identification Number (PIN)- See Authorization Code.
Person-to-Person Calls - An Operator Assisted call which is places under the stipulation that the caller will
speak only to a specific called party, a specified extension or office to be reached through a PBX attendant.
Such a call is not completed until either the specific party named by the caller is contacted, or the caller
agrees to speak to a different party. The caller must arrange with the operator to make a person-to-person
call; otherwise, all Operator Assisted calls will be treated as Operator Station calls.
Presubscribed Provider of Operator Services - The intrastate provider of Operator Services to which
the Authorized User is connected when the Authorized User places a call using a provider of operator
services without dialing a special access code.
Provider of Operator Services - Any common carrier that provides operator services or any other person
determined by the Federal Communications Commission and/or the Idaho Public Service Commission to be
providing operator services.
Real Time Rated - An intrastate call placed with the assistance of an operator, for which charges are
collected by an Aggregator, normally a hotel or motel, may be a hospital, from the guest or occupant of the
room from which the call originated. A call of this type requires that Dancris communicate the call detail and
charges back to the originating location following completion of the call. This service is provided only where
authorized by the Aggregator. Calls of this type are rated according to the Real Time Rate Schedules herein.

Renewal - A method of replenishing a Debit Account's Available Usage Balance with additional minutes of
usage as authorized and paid for by the Customer.
Sent Paid Coin - Sent paid coin rates apply to calls placed from pay telephone stations and paid for by
depositing coins at the pay telephone and are rated in real time. A call of this type requires Dancris to
communicate and collect the charges from the originating location.
Sponsor - A corporation or other legal entity that exclusively permits the use of its Marks to the Company
for use with telephone cards (prepaid or otherwise) or other merchandise, and contracts with the Company
for the marketing of the services described herein.
Switched Access - Where access between the Customer and the Carrier is provided on local exchange
company circuits capable of accessing the local switched network. The cost of switched Feature Group
access is billed to the Carrier.
Terminal Equipment - Devices, apparatus, and associated wiring, such as teleprinters, telephones, or data
sets.
Third Party Calls - An Operator Assisted call for which charges are billed not to the originating number,
but to a third party telephone number which is neither the originating nor the terminating telephone number.

SECTION 2 - RULES AND REGULATIONS
Undertaking of Dancris Telecom, L.L.
Dancris Telecom, LLC. ("Dancris ) services and facilities are furnished for communications
originating at specified points within the State of Idaho under terms of this tariff.
Dancris installs, operates and maintains the communications services provided herein in accordance
with the terms and conditions set forth under this tariff. Dancris may act as the Customer s agent for
ordering access connection facilities provided by other carriers or entities, when authorized by the
Customer, to allow connection of a Customer s location to the Dancris services. The Customer shall
be responsible for all charges due for such service arrangement.
The Company s services are available twenty-four (24) hours per day, seven (7) days per week.
Use
Service provided under this tariff may be used for any lawful purpose for which the service is
technically suited.

Limitations of Service

Service is offered subject to the continuing availability of the necessary facilities and/or
equipment and subject to the provisions of this tariff.
Dancris reserves the right to discontinue or limit service, upon appropriate notice, when
necessitated by conditions beyond its control, or when the Customer is using the service in
violation of the provisions ofthis tariff, or in violation oflaw.
The Company does not undertake to transmit messages, but offers the use of its facilities
when available, and will not be liable for errors in transmission or for failure to establish
connections.
All facilities provided under this tariff are directly or indirectly controlled by Dancris and
the customer may not transfer or assign the use of service or facilities without the express
written consent of the Company. Such transfer or assignment shall only apply where there
is no interruption of the use or location of the service or facilities.
Prior written permission from the Company is required before any assignment or transfer.
All regulations and conditions of service contained in this tariff shall apply to all such
permitted assignees or transferees.
Dancris reserves the right to discontinue the offering of any service with proper notice or
deny an application for service if a change in regulation materially and negatively impacts
the financial viability of the service in the best business judgment of the Company.

Liabilities of the Company

The liability of the Company for any claim or loss, expense or damage (including indirect
special, or consequential damage) for any interruption, delay, error, omission, or defect in
any service, facility or transmission provided under this tariff shall not exceed an amount
equivalent to the proportionate charges to the Customer for the period of service or the
facility provided during which such interruption, delay, error, omission, or defect occurs.
The Company shall not be liable for any claim or loss, expense, or damage (including
indirect, special, or consequential damage), for any interruption, delay, error, omission, or
other defect in any service facility, or transmission provided under this Tariff, if caused by
any person or entity other than the Company, by any malfunction of any service or facility
provided by any other carrier, by any act of God, fire, war, civil disturbance, or act of
government, or by any other cause beyond the Company s direct control.
The Company shall not be liable for, and shall be fully indemnified and held harmless by the
Customer against any claim or loss, expense or damage (including indirect, special or
consequential damage) for defamation, libel, slander, invasion, infringement of copyright
or patent, unauthorized use of any trademark, tradename or service mark, unfair competition
interference with or misappropriation or violation of any contract, proprietary or creative
right, or any other injury to any person, property or entity arising out of the material, data
information, or other content revealed to, transmitted, or used by the Company under this
tariff; or for any act or omission of the Customer; or for any personal injury or death of any
person caused directly or indirectly by the installation, maintenance, location, condition
operation, failure, presence, use or removal of equipment or wiring provided by the
Company, if not directly caused by negligence of the Company.
The Company shall not be liable for any claim, loss, or refund as a result of theft of a Debit
Card(s) or Personal Identification Numbers (PINs) issued for the use of the Company
services. Nor will the Company be liable for any claim, loss, or refund on any unused
balance remaining on a Debit Card provided to the Customer.

2.4.The Company shall not be liable for any claim, loss, or refund on any unused portion of the
credit balance remaining on a Debit Account provided to a Customer before or after the
expiration date assigned to each Debit Account or Card.
The included tariff language does not constitute a determination by the Commission that a
limitation of liability imposed by the Company should be upheld in a court of law.
Acceptance for filing by the Commission recognizes that it is a court's responsibility to
adjudicate negligence and consequential damage claims. It is also the court's responsibility
to determine the validity of the exculpatory clause.

2.5 Payment for Service
2.5.1 Payment Arrangements
The Customer is responsible for payment of all charges for services and equipment furnished
to the Customer for transmission of calls via the Company. This includes payment for calls
or services originated at the Customer's number(s) or incurred at the specific request of the
Customer. The Customer agrees to pay to the Company any cost(s) incurred as a result of
any delegation of authority resulting in the use of his or her communications equipment
and/or network services which result in the placement of calls via the Company. The
Customer agrees to pay the Company or its authorized agent any and all cost(s) incurred as
a result of the use of the service arrangement, including calls which the Customer did not
individually authorize.
All charges due by the Customer are payable to the Company or any agency duly authorized
to receive such payments. The billing agency may be the Company, a local exchange
telephone company, or other billing or commercial entity or service. Terms of payment shall
be according to the rules and regulations of the agency and subject to the rules of regulatory
agencies having jurisdiction. Any objections to billed or decremented charges must be
promptly reported to the Company or its billing agent. Adjustments to Customers' bills shall
be made to the extent that circumstances exist which reasonably indicate that such changes
are appropriate.
(C)Charges for third party calls which are charged to a domestic telephone number will
be included on the Billed Party's local exchange telephone company bill pursuant to
billing and collection agreements established by Dancris or its intermediary with the
applicable telephone company.
(D)Charges for credit card calls will be included on the Billed Party's regular monthly
statement from the card-issuing company.

(E)For Room Charge Calls (Time and Charges), when requested by the Authorized
User, and authorized by the Aggregator, the charges will be provided to the
Aggregator for inclusion on the hotel, motel, or hospital bill of the Authorized Users.
In such cases, Dancris will provide a record of the call detail and charges to the
hotel, motel, or hospital for such billing purposes. The Aggregator is solely
responsible for the collection of Room Charges from its guests, and remains liable
to Dancris for all Room Charge calls regardless of whether such charges are in fact
collected from the Authorized User. Room charge calls are rated in accordance
with the Real-Time Rate Table set forth in Section 3 herein.
(F)Charges for direct dialed calls will be included on the originating party's bill pursuant
to billing and collection agreements established by the Company or its intermediary
with the applicable telephone company.

Payment for Service (Cont'
Deposits
The Company reserves the right to examine the credit record of the Customer, using any
lawful sources for determining credit standing. If the Customer s financial condition is
unknown or unacceptable to the Company, the Customer may be required to provide the
Company with a security deposit which the Company may apply against overdue charges.
The amount of the security deposit shall not exceed two (2) month's estimated usage, may
vary with the Customer s credit history and projected usage, and be collected and maintained
in accordance with Commission rules. The Company may also require a security deposit
before service is restored, in addition to payment of overdue charges, from the Customer
whose service has been discontinued for nonpayment of overdue charges. The fact that a
security deposit has been made in no way relieves the Customer from the prompt payment
of bills upon presentation.
Advance Payments
For Customers whom the Company determines an advance payment is necessary, the
Company reserves the right to collect an amount not to exceed one (1) month's estimated
charges as an advance payment for service. This will be applied against the next month'
charges and a new advance payments may be collected for the next month.
The prepayment of services which are immediately available is a Debit Account Payment
and does not constitute an advance payment.

5.4 Payment Due Date and Late Payment Charges
All bills are due upon receipt. Any bill outstanding and unpaid more than thirty (30) days
after the date the bill is postmarked, shall be considered past due. A late payment fee of 1.
per month will be applied to any past due balance.
Return Check Charge
The Company reserves the right to assess a return check charge of$20.00 whenever a check
or draft presented for payment of service is not accepted by the institution on which it is
written. This charge applies each time a check is returned to the Company by a bank for
insufficient funds.
The Company reserves the right to place a hold on the increase of the Available Usage
Balance of a renewable Debit Card or the initial activation of any Debit Card until the check
or draft clears or is paid.
Billing Dispute
Any objections to billed charges must be reported to the Company or its billing
agent in writing within twenty (20) days of the closing date printed on the invoice
or statement issued to the Customer. Disputes may be submitted orally or in
writing. Adjustments to Customers' account shall be made to the extent that
circumstances exist which reasonably indicate that such changes are appropriate.
Late payment fees will not be applied during the period when a bill is disputed
regardless of the outcome of the dispute.

Taxes and Fees
The Company shall charge the Customer an amount sufficient to recover any governmental
assessments, fees, license, or other similar taxes or fees imposed upon the Company in support of
statutory or regulatory programs.
For Debit Card calls, Federal, State and local taxes and fees are included in the stated rates
in this tariff.
For all other services offered by the Company, taxes and fees shall be added pro-rata, insofar
as practical, to the rates and charges stated in the Company s rate schedules and listed as
separate line items on the Customer s bill for services provided.

Interruption of Service
Credit allowances for interruptions of service which are not due to the Company s testing or
adjusting, to the negligence of the Customer, or to the failure of channels, equipment or
communications systems provided by the Customer, are subject to the general liability provisions set
forth in herein. It shall be the obligation of the Customer to notify the Company immediately of any
interruption in service for which a credit allowance is desired by Customer. Before giving such
notice, the Customer shall ascertain that the trouble is not within his or her control, or is not in wiring
or equipment, if any, furnished by the Customer and connected to the Company s terminal.
Interruptions caused by Customer-provided or Company-provided automatic dialing equipment are
not deemed an interruption of service as defined herein.
Credit allowances for interruptions of service billed on a usage basis shall be limited to the rate
applicable to the initial period (if any) and surcharges (if any) of the call to compensate for re-
establishing the interrupted call.
Inspection, Testing and Adjustment
Upon reasonable notice, the facilities provided by the Company shall be made available to the
Company for such tests and adjustments as may be deemed necessary for maintenance in a condition
satisfactory to the Company. No interruption allowance will be granted for the time during which
such tests and adjustments are made.

Cancellation by Customer
Customers may cancel service at any time, either verbally or in writing. Customers are responsible
for all charges up through the actual disconnect date. Charges may be avoided by dialing another
carrier s access code. For prepaid services, the Customer may cancel service by fully depleting the
Available Usage Balance of the Debit Account and/or by not renewing a renewable account.
Interconnection
Service furnished by Dancris may be connected with the services or facilities of other carriers. Such
service or facilities, if used, are provided under the terms, rates and conditions of the other carrier.
The Customer is responsible for all charges billed by other carriers for use in connection withDancris.'s service. Any special interface equipment or facilities necessary to achieve compatibility
between carriers is the responsibility of the Customer.
Terminal Equipment
Company s facilities and service may be used with or terminated in Customer-provided terminal
equipment or systems, such as PBXs, key systems, multiplexers, repeaters, signaling setsteleprinters, handsets, or data sets. Such terminal equipment shall be furnished and maintained at the
expense of the Customer, except as otherwise provided. The Customer is responsible for all costs at
his or her premises, including personnel, wiring, electrical power, and the like, incurred in the useof Company s service.
Cost of Collection and Repair
The Customer is responsible for any and all costs incurred in the collection of monies due the
Company including legal and accounting expenses. The Customer is also responsible for recovery
costs of Company-provided equipment and any expenses required for repair or replacement of
damaged equipment.

Refusal or Discontinuance by Company
The Company may refuse or discontinue service under the following conditions provided that, unless
otherwise stated, the Customer shall be given proper notice to comply with any rule or remedy any
deficiency:
F or non-compliance with or violation of any State, municipal, or F ederallaw, ordinance or
regulation pertaining to telephone service.
For use of telephone service for any purpose other than that described in the application.
For neglect or refusal to provide reasonable access to the Company or its agents for the
purpose of inspection and maintenance of equipment owned by the Company or its agents.
For noncompliance with or violation of Commission regulation or rules and regulations on
file with the Commission.
For non-payment of any amount past due to the Company by the Customer.
Without notice in the event of the Customer or Authorized User use of equipment in such
a manner as to adversely affect the Company ' equipment or service to others.

Refusal or Discontinuance by Company, (Cont'
Without notice in the event of tampering with the equipment or services owned by the
company or its agents.
Without notice in the event of unauthorized or fraudulent use of service. Whenever service
is discontinued for fraudulent use of service, the Company may, before restoring service
require the Customer to make, at his or her own expense, all changes in facilities or
equipment necessary to eliminate illegal use and to pay an amount reasonably estimated as
the loss in revenues resulting from such fraudulent use.
Without notice by reason of any order or decision of a court or other government authority
having jurisdiction which prohibits the Company from furnishing such services.
When the Available Account Balance of a non-renewable account is depleted to a level
insufficient to place a one-minute call to the location of least cost.
When the established expiration date ofthe Debit Account is reached.

Tests, Pilots, Promotional Campaigns and Contests
The Carrier may conduct special tests or pilot programs and promotions at its discretion to
demonstrate the ease of use, quality of service and to promote the sale of its services. The Carrier
may also waive a portion or all processing fees or installation fees for winner of contests and other
occasional promotional events sponsored or endorsed by the Carrier. From time to time the
Company may waive all processing fees for a Customer.
Installation and Termination
Service is installed upon mutual agreement between the Customer and the Company. The service
agreement does not alter rates specified in this tariff
Other Rules
The Company may temporarily suspend service without notice to the Customer, by blocking traffic
to certain cities or NXX exchanges, or by blocking calls using certain Personal Identification
Numbers when the Company deems it necessary to take such action to prevent unlawful use of its
service. The Company will restore service as soon as service can be provided without undue risk.
Individual Case Basis (ICB) Arrangements
Arrangements will be developed on a case-by-case basis in response to a bona fide request from a
Customer or prospective Customer to develop a competitive bid for a service offered under this tariff.
Rates quoted in response to such competitive requests may be different than those specified for such
services in this tariff. ICB rates will be offered to the Customer in writing and on a
nondiscriminatory basis.

2.18 Responsibilities of Aggregators
In addition to the responsibilities of Aggregators in their capacities as Subscribers, Aggregators must
also adhere to the following requirements:
2.18.1 Aggregators must post on the telephone instrument, in plain view of Authorized Users:
(A)The name, address, and toll-free telephone number of the provider of operator
services; and
(B)A written disclosure that the rates for all operator-assisted calls are available on
request, and that Authorized Users have a right to obtain access to the intrastate
common carrier of their choice and may contact their preferred intrastate common
carriers for information on accessing that carrier's service using that telephone; and
(C)The name and address of the enforcement division the Federal Communications
Commission, to which the Authorized User may direct complaints regarding
Operator Services; and
(D)Any other information required by state or federal regulatory agencies or law.
2.18.2 Aggregators must ensure that each of its telephones presubscribed to a provider of operator
services allows the Authorized User to use "800" and "950" access code numbers to obtain
access to the provider of operator services desired by the Authorized User.

2.18.3 Dancris shall withhold service (on a location-by-location basis) of any compensation,
including commissions, to Aggregators if Dancris reasonably believes that the Aggregator
(i) is blocking access by means of "950" or "800" numbers to intrastate common carriers in
violation of The Telephone Consumer Protection Act of 1990 paragraph 3.4.1.B.; or (ii) is
blocking access to equal access codes in violation of rules the Federal Communication
Commission and/or the state Commission may prescribe.
2.19 Responsibilities of the Subscriber
2.19.1 The Subscriber is responsible for placing any necessary orders, for complying with tariff
regulations, and for ensuring that Authorized Users comply with tariff regulations. The
Subscriber is also responsible for the payment of charges for calls originated at the
Subscriber's premises that are not collect, third party, calling card, or credit card calls.
2.19.2 The Subscriber is responsible for charges incurred for special construction and/or special
facilities that the Subscriber requests and which are ordered by Dancris on the Subscriber's
behalf.
2.19.3 If required for the provision of Dancris’ Services, the Subscriber must provide any
equipment space, supporting structure, conduit, and electrical power without charge to
Century.
2.19.4 The Subscriber is responsible for arranging ingress to its premises at times mutually
agreeable to it and Dancris when required for Dancris personnel to install, repair, maintain,
program, inspect, or remove equipment associated with the provision of Dancris’ Services.

2.19.5 The Subscriber shall ensure that its terminal equipment and/or system is properly interfaced
with Dancris’ facilities or services, that the signals emitted into Dancris’ network
configuration are of the proper mode, bandwidth, power, and signal level for the intended use
of the Subscriber and in compliance with the criteria set forth in Part 68 of the Code of
Federal Regulations, and that the signals do not damage equipment, injure personnel, or
degrade service to other Subscribers.
2.19.6 If the Subscriber fails to maintain the equipment and/or the system properly, with resulting
imminent harm to Dancris’ equipment, personnel, or the quality of Service to other
Subscribers or Customers, Dancris may, upon written notice, require the use of protective
equipment at the Subscriber's expense. If this fails to produce satisfactory quality and safety,
Dancris may, upon written notification, terminate the Subscriber's service.
2.19.7 The Subscriber must pay Dancris for replacement or repair of damage to the equipment or
facilities of Dancris caused by negligence or willful act of the Subscriber, its Authorized
Users, or others, or by improper use of equipment provided by the Subscriber, Authorized
Users, or others.
2.19.8 The Subscriber must pay for the loss through theft or fire of any of Dancris’ equipment
installed at Subscriber's premises.

2.20 Responsibilities of Authorized Users
2.20.1 The Authorized User is responsible for compliance with the applicable regulations set forth
in this tariff as well as all rules and regulations of the state utility commission and the FCC.
2.20.2 The Authorized User is responsible for identifying the station, party, or person with whom
communication is desired and/or made at the called number.
2.20.3 The Authorized User is responsible for providing Dancris with a valid method of billing for
each call. Dancris reserves the right to validate the credit worthiness of users through
available credit card, calling card, called number, third party telephone number, and room
number verification procedures. Where a requested billing method cannot by validated, the
user may be required to provide an acceptable alternate billing method or Dancris may
refuse to place the call.

3.1 General
3.1.1 The Company provides telecommunications services between locations within the State of
Idaho as specified herein. The Company's service charges may be based upon call duration,
time of day rate period, mileage, and/or call type.
3.1.2 Presubscribed service is offered from locations served with equal access end offices.
3.1.3 The Company's service is available twenty-four hours per day, seven days a week.
3.1.4 Dancris’ Operator Service is provided for use by presubscribed Customers as well as
transient Customers at host or Subscriber locations. Services arranged for the use of the
transient public are subject to restrictions imposed by the Idaho Public Service Commission
and the Federal Communications Commission.

Calculation of Distance
Usage charges for any mileage sensitive services are based on the airline distance between
the rate center locations associated with the originating and terminating points of the call.
The distance between the originating and terminating points is calculated by using the "
and "H" coordinates of the applicable rate centers as defined by BellCore (Bell
Communications Research), in the following manner:
Step 1 - Obtain the "V" and "H" coordinates for the rate center of the originating and the
destination points.
Step 2 - Obtain the difference between the "V" coordinates of each of the rate centers.
Obtain the difference between the "H" coordinates.
Step 3 - Square the differences obtained in Step 2.
Step 4 - Add the squares of the "V" difference and "H" difference obtained in Step 3.
Step 5 - Divide the sum of the square obtained in Step 4 by ten (10). Round to the next

3.3 Timing of Calls
Billing for calls placed over the network is based in part on the duration of the call.
3.3.1 For Direct Dialed and Operator Station calls, chargeable time begins when the called party
answers the call (i.e. when two way communications are established.) Answer detection
is based on standard industry answer detection methods, including hardware and software
answer detection. For Person-to-Person calls, chargeable time begins when the designated
party comes on the line, or when the caller agrees to speak with a substitute party.
3.3.2 Chargeable time for all calls ends when one of the parties disconnects from the call. If the
called party hangs up but the calling party does not, chargeable time ends when the network
connection is released by automatic timing equipment (or, in some instances, when the calling
party initiates another call without hanging up).
3.3.3 Minimum call duration for billing purposes is thirty (30) seconds unless otherwise specified
in the individual rate schedules of this tariff. Any partial thirty second increment is rounded
up to a full increment.
3.3.4 Calls are measured and billed in six (6) second increments unless otherwise specified in the

Travel Card Service
Travel Card Service is available in conjunction with other Dancris services, or as a stand-
alone offering. The service is typically used for originating telephone calls while away from
home or office. Service is accessed by dialing the Company-designated toll-free access
number, a valid authorization code, and the destination number. Calls are billed in sixty (60)
second increments following a minimum billing period of sixty (60) seconds. Intrastate

Pre-Paid Calling Card
Dancris' Pre-Paid Card plans allow customers to pay a fixed dollar amount in advance for
long distance calling. Customers must dial an access code from a touch tone phone to use
the service. Cards are rated in dollars per minute. Fractional call minutes are rounded up
to the nearest full minute. As calls are placed, charges for the call are deducted on a real-
time basis until the full amount ofthe card is exhausted. Calls are limited to direct dial calls.
Air to ground services, as well as calls to 500, 700, or 900 numbers are blocked. (See
Section 4.2 regarding the Public Telephone Surcharge.
Cards are available in replenishable and non-replenishable varieties. Replenishable cards
expire 180 days from last use. Non-replenishable cards expire one (I) year from the date of
purchase, or 180 days from last use. The fee for replenishment is $1.00 per occasion per
account.
The Dancris Pre-Paid Calling Card plans are available for use twenty- four hours a day, seven
days a week. Long distance taxes are included in the purchase price of the card.
A Pre-Paid Calling Card account is established upon receipt of payment by the Company.
The Company reserves the right to determine acceptable types of payment.
Enhanced calling features, such as Voice Mail, Fax Store and Forward, Fax Broadcast
Conference Calling and Voice Recognition may be available with Dancris' Pre-Paid calling
card services. These services may involve additional charges.

Directory Assistance
A Directory Assistance charge applies per intrastate directory assistance call. The customer
may make two (2) requests for a telephone number per call. The Directory Assistance
charge applies regardless of whether the operator is able to supply the requested number.

Public Telephone Surcharge
In order to recover the Company s expenses to comply with the FCC's pay telephone
compensation plan effective on October 7, 1997 (FCC 97-371), an undiscountable per call
charge is applicable to all interstate, intrastate, and international calls that originate from any
domestic pay telephone used to access Dancris services. This surcharge, which is in addition
to standard tariffed usage charges and any applicable service charges and surcharges
associated with Dancris service, applies for the use of the instrument used to access Dancris
service and is unrelated to the Dancris service accessed from the pay telephone.
Pay telephones include coin-operated and coinless phones owned by local telephone
companies, independent companies and other interexchange carriers. The Public Pay
Telephone Surcharge applies to the initial completed call and any reoriginated call (e.
using the "#" symbol). The Public Pay Telephone Surcharge does not apply to calls placed
from pay telephones at which the Customer pays for service by inserting coins during the
progress of the call.
Whenever possible, the Public Pay Telephone Surcharge will appear on the same invoice
containing the usage charges for the surcharged call. In cases where proper pay telephone
coding digits are not transmitted to the Company prior to completion of a call, the Public Pay
Telephone Surcharge may be billed on a subsequent invoice after the Company has obtained
information from a carrier that the originating station is an eligible pay telephone.

SECTION 4 - PROMOTIONS
Promotions - General
From time to time the Company may, at its option, promote subscription or stimulate network usage
by offering to waive some or all of the nonrecurring or recurring charges for the Customer (if
eligible) of target services for a limited duration, not to exceed 90 days, or by offering premiums or
refunds of equivalent value. Such promotions shall be made available to all similarly situated
Customers in the target market area. The Company shall notify the Commission, in writing, 30 days
prior to the start of a promotion.
Demonstration Calls
From time to time Dancris will demonstrate its services by providing free test calls of up to fifteen
minutes duration over its network.

SECTION 5 - CONTRACT SERVICES
5.1 General
At the option of the Company, service may be offered on a contract basis to meet specialized
requirements of the Customer not contemplated in this tariff. The terms of each contract shall be
mutually agreed upon between the Customer and Company and may include discounts off of rates
contained herein, waiver of recurring or nonrecurring charges, charges for specially designed and
constructed services not contained in the Company’s general service offerings, or other customized
features. The terms of the contract may be terminating access, mixture of services or other
distinguishing features. Service shall be available to all similarly situated Customers for six months
after the initial offering to the first contract Customer for any given set of terms.

Travel Card Service is available in conjunction with other Grandfathered services. The service is
typically used for originating telephone calls while away from home or office. Service is accessed
by dialing the Company-designated toll-free access number, a valid authorization code, and the
destination number. Calls are billed in sixty (60) second increments following a minimum billing
period of sixty (60) seconds.
