SECTION 1 - TERMS AND ABBREVIATIONS
Access Line - A local channel for voice, data, or video communications which connects the
Customer location to a location of the Company.
Account - The Customer who has agreed, verbally or by signature, to honor the terms of
service established by the Company. An account may have more than one access code billed
to the same Customer address.
Authorization Code - A pre-defined series of numbers to be dialed by the Customer or End
User upon access to the Company s system to notify the caller and validate the caller
authorization to use the services provided. The Customer is responsible for charges incurred
through the use of his or her assigned Authorization Code.
Calling Card - A billing convenience whereby the End User may bill the charges for a call
to an approved local exchange company-issued Calling Card. The terms and conditions of
the local exchange company shall apply to payment arrangements.
Clear World - Used throughout this price list to refer to Clear World Communications
Corporation unless otherwise indicated by the text.
Commission - Refers to the Idaho Public Utility Commission.
Company - Clear World Communications Corporation , unless stated otherwise.
Company s Point of Presence - Location of the serving central office associated with
access to the Company s network.
Customer - Any person, firm, partnership, corporation or other entity which uses service
under the terms and conditions of this tariff and is responsible for the payment of charges.
Customer Dialed Calling Card Call- A service whereby the End User dials all ofthe digits
necessary to route and bill the call.
Dedicated Access - A method of reaching the Company s services whereby the Customer
is connected directly to the Company s Point of Presence without utilizing services of the
local switched network.

End User - Any person, firm, partnership, corporation or other entity which uses the service
of the Company under the terms and conditions of this tariff. The End User is responsible
for payment unless the charges for the service utilized are paid by the Customer.
Equal Access - A form of dialed access provided by local exchange companies whereby
interexchange calls dialed by the Customer are automatically routed to the Company
network. Presubscribed Customers may also route interexchange calls to the Company
network by dialing an access code supplied by the Company.
Holidays - New Year s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving
Day, and Christmas Day.
Initial And Additional Period - The Initial Period denotes the interval of time allowed at
the rate specified for a connection between given service points. The Additional Period
denotes the interval of time used for measuring and charging for time in excess of the Initial
Period.
LATA - Local Area of Transport and Access
Special Access - See Dedicated Access.
Switched Access - A method for reaching the Company through the local switched network
whereby the End User uses standard business or residential local lines.
Terminal Equipment - Telecommunications devices, apparatus and associated wiring on
the Premises of the Customer.
Travel Card - A billing mechanism which enables the Customer to access the service ofthe
Company while away from home or office.

Undertaking of Clear World Communications Corporation
Clear World's services and resold facilities are furnished for intraLATA and
interLA T A communications originating at specified points within the state ofldaho
under terms of this price list.
Clear World installs, operates, and maintains the communications services provided
hereinunder in accordance with the terms and conditions set forth under this price
list. Clear World may act as the Customer s agent for ordering access connection
facilities provided by other carriers or entities, when authorized by the Customer, to
allow connection of a Customer s location to the Clear World network. The
Customer shall be responsible for all charges due for such service arrangement.
The Company s services and resold facilities are provided on a monthly basis unless
otherwise provided, and are available twenty-four hours per day, seven days per
week.
Limitations
Service is offered subject to the availability of the necessary facilities and
equipment and subject to the provisions of this price list.
Clear World reserves the right to discontinue or limit service when
necessitated by conditions beyond its control, or when the Customer is using
service in violation of provisions of this price list, or in violation ofthe law.
The Company does not undertake to transmit messages, but offers the use of
its facilities when available, and will not be liable for errors in transmission
or for failure to establish connections.

Limitations, (Cont'
All services and resold facilities provided under this price list are directly or
indirectly controlled by Clear World and the Customer may not transfer or
assign the use of service or facilities without the express written consent of
the Company. Such transfer or assignment shall only apply where there is no
interruption of the use or location of the service or facilities.
Prior written permission from the Company is required before any
assignment or transfer. All regulations and conditions contained in this price
list shall apply to all such permitted assignees or transferees, as well as all
conditions of service.
Pre subscribed service is available from equal access locations only. Travel
service is available from any originating location in the state.
The included price list language does not constitute a determination by the
Commission that a limitation ofliability imposed by the Company should
upheld in a court of law. Acceptance for filing by the Commission
recognizes that it is a court's responsibility to adjudicate negligence and
consequential damage claims. It is also the court's responsibility to determine
the validity of the exculpatory clause.
Use
Services provided under this price list may be used for any lawful purpose for which
the service is technically suited.

Payment for Service
All charges due by the Customer are payable to Clear World or any agency duly
authorized to receive such payments. Any objections to billed charges must be
reported to the Company within thirty (30) days of the invoice date. Adjustments to
Customer s bills shall be made to the extent that circumstances exist which
reasonably indicate that such changes are appropriate.
Customers are responsible for all charges associated with their account, including all
charges placed against Travel Card numbers. Customers claiming not to be
responsible for more than five calls on anyone statement may be required to accept
a Travel Card number change issued by Clear World.
Taxes
All state and local taxes (i., gross receipts tax, sales tax, municipal utilities tax) are
listed as separate line items and are not included in the quoted rates.
Cancellation
Customers must provide thirty days written notification to HLC prior to cancellation.
Customers are responsible for all charges, including fixed fees, which accrue up to
the cancellation date.

Other Rules
The Company reserves the right to discontinue service, limit service, or to impose
requirements on Customers as required to meet changing regulatory rules and
standards of the Idaho Public Utilities Commission.
Refunds or Credits for Service Outages or Deficiencies
Credit allowances for interruptions of service caused by service outages or
deficiencies are limited to the initial minimum period call charges for re-establishing
the interrupted call.
800 Numbers
The Company will make every effort to reserve "800" vanity numbers on
behalf of customers, but makes no guarantee or warrantee that the requested
800" number(s) will be available or assigned to the customer requesting the
number.
2 If a Customer accumulates undisputed past-due charges, the Company
reserves the right not to honor the Customer s request for a change in 800
service to another carrier (e.

porting" of the 800 number), including a
request for a Responsible Organization (Resp. arg.) change, until such time
as all charges are paid in full.
800 numbers shared by more than one Customer, whereby individual
customers are identified by a unique Personal Identification Number, may not
be assigned or transferred for use with service provided by another carrier.
Subject to the limitations provided in Section 2., the Company will only
honor Customer requests for change in Resp. Org. or 800 service provider for
800 numbers dedicated to the sole use of that single Customer.

Calculation of Distance
U sage charges for mileage sensitive services vary based on the type of service
subscribed to by the Customer. For services utilizing switched access, mileage
measurements for rate schedules are based on the distance in airline miles between
rate centers associated with the originating and terminating stations. For services
utilizing dedicated access, mileage measurements for rate schedules are based on the
distance in airline miles between the Clear World network access point associated
with the station utilizing Dedicated Access Origination/Termination and the rate
center associated with the called/calling station.
The distance between the originating and terminating points is calculated by using
the "V" and "H" coordinates ofthe serving wire centers as defined by BellCore (Bell
Communications Research), in the following manner:

Obtain the "V" and "H" coordinates for the serving wire center or
network access point serving the Customer s location and the
called/calling station.
Obtain the difference between the "V" coordinates. Obtain the

Step 3.
Divide the sum ofthe square obtained in Step 4 by ten (10). Round
to the next higher whole number if any fraction results from the
division.

Timing of Calls
Billing for calls placed over the network is based in part on the duration of the call.
Timing for all calls begins when the called party answers the call (i.e. when
two way communications are established.) Answer detection is based on
standard industry answer detection methods, including hardware and software
answer detection.
Chargeable time for all calls ends when one of the parties disconnects from
the call.
Call durations and minimum calling periods are provided with each specific
product as described in this tariff.
There is no billing applied for incomplete calls.

Rate Periods
Unless otherwise indicated elsewhere in this tariff, all usage-based rates are subject
to the following time-of-day and day-of-week:
Day Rate Period - Applies to that portion of a call occurring from 8:00 AM
, but not including, 5:00 PM Monday through Friday.
Evening Rate Period - Applies to that portion of a call occurring from 5:00
PM to, but not including, 11 :00 PM Sunday through Friday.
Night/Weekend Rate Period - Applies to that portion of a call occurring from
11:00 PM to, but not including 8:00 AM all days, 8:00 AM to, but not
including, 11 :00 PM Saturday; 8:00 AM to, but not including, 5:00 PM
Sunday.
3.4.When a call is established in one rate period and ends in another rate period
the rate in effect at the calling station applies to the portion of the call
occurring within that rate period. When a unit of time is split between two
rate periods, the rate applicable to that unit of time is based on the rate period
in which it began.

3.34 Clear World Unlimited Plan 61

Clear World Unlimited Plan 61 is a direct dial outbound service for residential customers. Customers
will receive unlimited intrastate long distance voice usage. Customers will be billed a monthly
recurring charge (some restrictions may apply as outlined below) for this service. This plan is only
available to Customers who also subscribe to the Residential Service Plan offered by the Company’s
affiliated local exchange entity. This plan does not offer call Detail, but if the Customer request Call
Detail, the rates are outlined below.

This service is only offered in conjunction with the corresponding interstate Clear World Unlimited
Plan 61.

3.34.1 Rates and Charges

Monthly Recurring Charge: $24.95

Call Detail: $3.00

3.34.2 Customer Restrictions

New and existing Customers are eligible for this service if they meet the following
requirements:

(A) The Company reserves the right to move a Customer on this plan to an alternative
plan if the Company determines, in its sole discretion, that the Customer’s usage on
the plan does not resemble typical residential usage.

(B) Customers must presubscribed to Clear World Communications Corporation for
both intraLATA and interLATA long distance services.

(C) This plan cannot be used for any use inconsistent with residential service.

(D) This plan is not available to Customers with a multi-line account or an account that
bills to another number or is the recipient of charges billed from another number
unless the Customer establishes separate billing accounts for each line.

(F) Unlimited plan usage does not include calls used for general business purposes,
multi-party conference calls, calls to 900 numbers, directory assistance, calling card,
operator services, international calling, commercial facsimile, auto-dialing, call
centers and/or direct telemarketing centers. Calls to Residential Toll-Free accounts
are not included in this Unlimited plan.

(G) In order to be eligible for this plan, the Company must be able to verify that the
Customer meets these eligibility requirements. Customers who no longer meet these
eligibility requirements will not be eligible for this plan and without prior notice will
be placed on an alternative plan or will have their service suspended, restricted or
canceled.

(H) The monthly rate for this plan does not include applicable taxes, surcharges or fees.

(I) This plan is not available for resale.

(J) Customers failing to pay the monthly recurring charge will result in calls being
billed at Clear World through the local telephone company. Clear World reserves
the right to disconnect any customer who is delinquent in their payments of over 60
days.

Directory Assistance charge applies to each call to the Directory Assistance Bureau.

Bureau is able to furnish the requested telephone number.
